About this file
This file is a required sworn statement form pursuant to Kentucky Revised Statute (KRS) 45A.395 for bidders or offerors responding to contracting opportunities with the City of Owensboro, Kentucky. The document serves as a prerequisite compliance form that must be submitted to establish bidder responsibility, requiring the signatory to affirm under penalty of perjury that neither they nor anyone acting on behalf of their organization has knowingly violated Kentucky's campaign finance laws, and that awarding a contract to their organization would not violate these laws. The form includes blank fields for completion in 2025, with spaces for company name, authorized representative signature, and title.
The statement defines "knowingly" as being aware or should have been aware that conduct violates campaign finance statutes, emphasizing the legal accountability of the declaration. This compliance document appears to be a standard requirement for all procurement processes with the City of Owensboro, located at 101 E. 4th Street, and can be contacted through their Purchasing Department at 270-687-8431. The form represents a regulatory safeguard to ensure ethical procurement practices and prevent conflicts of interest related to campaign contributions in the Commonwealth of Kentucky's public contracting process.
