About this file
The document is a Supplier Responsibility Questionnaire from Lead Public Schools, designed to assess a potential contractor's qualifications for doing business in Tennessee. The questionnaire is a comprehensive evaluation tool that examines a supplier's legal business entity information, integrity, legal proceedings, and financial and organizational capacity over the past five years. It requires detailed responses across four main sections, including verification of the supplier's legal status, any history of suspension or debarment, involvement in legal investigations, and past contract performance.
The questionnaire seeks to determine a supplier's eligibility by requiring transparency about potential issues such as contract terminations, liens, judgments over $25,000, unsatisfactory performance assessments, and pending litigation. Suppliers must disclose any incidents that might impact their ability to fulfill contract requirements, including suspensions, legal proceedings, financial challenges, or previous contract performance problems. The document serves as a critical pre-qualification step to ensure that potential contractors have the appropriate legal authority, financial stability, operational capacity, and integrity to enter into a business relationship with Lead Public Schools.
