If the respondent answers affirmatively, they must provide detailed conviction information including court/jurisdiction, docket/case number, statute/charge, and date of sentence. The form requires the respondent's printed name, signature, title, and date, along with an attestation under penalty of perjury that all provided information is true, correct, complete, and made in good faith. False or fraudulent information may result in disqualification from employment consideration or adverse action up to removal, and constitutes a criminal offense under 18 USC 1001.
