About this file

The document is a Past Performance Questionnaire for a Federal Emergency Management Agency (FEMA) Request for Quote (RFQ) 70FBR425Q00000162 for Level II Armed Security Guard Services in Florida. The questionnaire is designed to collect performance feedback from previous contractors, with evaluators asked to rate the contractor's performance across three key elements: staffing capacity, customer satisfaction, and responsiveness/attentiveness using a scale from Superior to Unsatisfactory. The form requires confidential submission directly to the FEMA Contracting Officer by the closing date, with ratings to be used in evaluating the offeror's past performance for a small business set-aside contract supporting disaster response and recovery operations in Florida.

The solicitation is specifically for armed security guard services at temporary and fixed facilities including Joint Field Offices, staging yards, and branch offices, with a preference for local firms in counties affected by Hurricane Helene and Hurricane Milton. The contract is set aside for small businesses under NAICS code 561612 with a size standard of $29M, and funding is contingent on appropriation availability. The procurement emphasizes local business participation in support of disaster recovery efforts, with potential contractors required to demonstrate their previous performance capabilities through this detailed questionnaire process.
