About this file

This is an application response form from the Massachusetts Department of Mental Health (DMH) for a Request for Application (RFA) #2026-DMH-3033-01 titled "Camps for Department of Mental Health Referred Children Master Agreement". The RFA seeks to establish a contract for camp services for children referred by the department, with providers offering both day and overnight camp options. Bidders are required to submit their applications through the COMMBUYS online procurement system, with specific formatting and submission guidelines. The document indicates that this is for a camp services contract, likely for the upcoming 2026 camp season.

The application requires detailed information from potential camp providers, including organizational type, fiscal solvency, liability insurance, licensing documentation, and compliance certifications. Bidders must provide proof of Department of Public Health licensing, camp brochures, and demonstrate experience operating camps for at least three consecutive years. The DMH recommends an all-inclusive weekly rate but will consider alternative pricing structures. Applicants must also complete various supplementary documents, such as an Environmental Preferable Products Checklist, Supplier Diversity Plan, and a Certification of Assurance of Compliance related to accessibility and non-discrimination requirements for persons with disabilities.
