About this file

This document is a Vendors Certification form for Halifax Health, specifically Attachment D addressing scrutinized companies under Florida Statutes. The certification requires vendors to affirm they are not on specific lists of companies that boycott Israel, have activities in Sudan or Iran's petroleum sector, or conduct business operations in Cuba or Syria. Vendors must disclose their company information, including name, federal identification number, authorized representative details, address, contact information, and provide a sworn certification about their compliance with state procurement restrictions.

The document outlines significant contractual consequences for false certifications, including potential civil penalties, attorney's fees, and contract termination. For contracts under $1 million, Halifax Health can terminate the agreement if a vendor is found to have submitted a false certification or is engaged in boycotting Israel. For contracts of $1 million or more, additional grounds for termination include being listed on Sudan or Iran petroleum sector lists or conducting business operations in Cuba or Syria. The certification serves as a legal mechanism to ensure vendor compliance with Florida's ethical procurement guidelines regarding international business practices.
