About this file

This is an Anti-Human Trafficking Affidavit form required by Broward County, Florida for the Well Column Pipes contract opportunity. The affidavit must be completed and submitted with the vendor's proposal, or if not included initially, within three business days of the County's request, upon contract award, renewal, or extension. Vendors who fail to comply with this requirement within the stated timeframe may be deemed nonresponsive. The form requires the vendor to declare under penalties of perjury that they do not use coercion for labor or services as defined in Section 787.06, Florida Statutes, and must be signed by an authorized representative with printed name, title, and date.

The affidavit serves as a compliance certification rather than a pricing or technical document. Vendors must acknowledge their adherence to anti-human trafficking standards as a condition of doing business with Broward County. This requirement applies to all contract submissions, including the open-end procurement for Well Column Pipes, which covers the purchase and delivery of new pipes for the County's Water and Wastewater Services and other agencies. All vendor submissions must be made electronically through BPRO by the specified closing date and time, with the completed affidavit included to ensure full compliance and responsiveness to the solicitation requirements.
