About this file

This is an Anti-Human Trafficking Affidavit form required by Broward County, Florida for vendors submitting bids or renewing contracts. The affidavit must be completed and submitted with the vendor's solicitation response for the Utility Bill Collection Services contract. If the form is not provided at the time of submittal, vendors are required to submit it within three business days of the County's request and upon contract award, renewal, or extension. Vendors must declare under penalties of perjury that they do not use coercion for labor or services as defined in Section 787.06, Florida Statutes. The form requires the vendor to provide their company name, authorized representative's signature, printed name, title, and submission date. Failure to fully comply with submission requirements within the stated timeframe may result in the vendor being deemed nonresponsive.

This affidavit is a compliance requirement rather than a pricing or technical document. It serves as a mandatory certification that vendors must provide to demonstrate their commitment to anti-human trafficking standards and ethical labor practices. The form, dated September 9, 2024, is a standard document in Broward County's procurement process and applies to all contracts including the open-end Utility Bill Collection Services contract for Water and Wastewater Services and other County agencies. Vendors must submit all required documentation electronically through BPRO (Bonfire) by the specified closing date and time to be considered for award.
