About this file

This Cost Proposal Form pertains to the Utah Department of Transportation's Request for Proposal for a Utah Broadband BEAD Consultant to support the Utah Broadband Center in federal grant administration and compliance under the Broadband Equity, Access, and Deployment Program. The solicitation anticipates a single contract award with the selected consultant providing expert guidance on Federal Grant Administration (2 CFR 200) and telecommunications/broadband regulatory issues. The consultant team will assist the Utah Broadband Center in its capacity as the Eligible Entity and Pass-Through Entity for administering the BEAD Program.

Offerors must submit pricing proposals using the prescribed cost form without modification, providing a blended hourly rate that represents the maximum billable rate for all services including labor costs and profit. Additionally, offerors must specify separate labor rates for three position categories: Senior Manager/Executive, Mid-Level Staff, and Support Staff. Cost evaluation will be based on a formula where the lowest proposal receives 100 percent of available cost points, while all other proposals receive points proportionally based on their percentage above the lowest price, with proposals exceeding double the lowest price receiving no points. Travel costs will be reimbursed at the State of Utah's current per diem rate according to state travel policy, and the resulting contract will authorize payment for labor hours at agreed-upon rates plus pre-authorized pass-through direct expenses with invoices.

Cost Proposal

To be submitted as a separate document from the Technical Response document Offeror Name:

Cost is to be submitted based on the following:

(Any deviation from this format may result in disqualification of proposal.)

Provide a blended hourly rate for the tasks described in the RFP. The “blended” hourly rate is the maximum billable rate for all services and must include all labor costs for staff, and profit. Labor may include account supervision, planning, consultation, and other labor expenses required to accomplish the needed tasks.

Blended Hourly Rate:
$________ per/hr.

Note: The Blended Hourly Rate will be used as the basis for assigning Cost Points in the RFP evaluation. The Position specific labor rates are for contracting purposes.

Additionally, provide the following specific position labor rates:

Position
Associated Rate
1. Senior Manager/ Executive
$
2. Mid-Level Staff
$
3. Support Staff
$

Purchasing will use the following cost formula: The points assigned to each offeror’s cost proposal will be based on the lowest proposal price. The offeror with the lowest Proposed Price will receive 100% of the price points. All other offerors will receive a portion of the total cost points based on what percentage higher their Proposed Price is than the Lowest Proposed Price. An offeror whose Proposed Price is more than double (200%) the Lowest Proposed Price will receive no points. The formula to compute the points is: Cost Points x (2- (Proposed Price/Lowest Proposed Price)).

IMPORTANT: Offerors may not change/modify the format of this cost form. The State relies on this cost sheet to accurately and fairly compare offeror costs. Cost Proposals received in a modified or alternate format may be disqualified.

*Costs of travel will be reimbursed at the then-current per diem rate set by the State of Utah Travel Policy, as found at: http://www.rules.utah.gov/publicat/code/r025/r025-007.htm The resultant contracts will specifically authorize payment for time spent on work based on the agreed upon labor rates, it also is the intent of the contract to allow for the payment of pass-through direct expenses (accompanied with invoices) as pre-authorized by the project manager.
