The affidavit is a legal document designed to certify that the bid being submitted is genuine and not the result of collusive practices. The form requires the bidder to swear under oath that they have not induced or solicited other firms to submit sham bids, nor have they sought to gain an unfair advantage over other bidding firms.
The affidavit must be completed, signed, and notarized, with the notary public documenting the date of attestation. The form includes blank spaces for the firm's name, the individual's name and signature, their title, and the notary's details.
