About this file
This document is an amendment to an existing agreement between the Massachusetts Department of Public Health and a vendor for the provision of allergen training video and certification services. The amendment outlines revised procedures for corporate clients who wish to engage the vendor to provide training for their employees. Specifically, the corporation may provide corporate contact information and employee ID numbers in lieu of personal identifying information, and the employee certificates will remain valid as long as the employee is employed by the corporation, up to a maximum of five years. If the employee leaves the corporation before the five-year expiration, the certificate will become void and the individual will need to be recertified.
The related state and local contract opportunity is for an online training and certification site for allergen awareness, which is located in the state of Massachusetts. No additional details about the contract opportunity, such as quantities, response dates, or award information, are provided in the given documents.
