About this file

This is an ATF E-Form 8620.42 Police Check Inquiry form used by the Bureau of Alcohol, Tobacco, Firearms and Explosives (ATF) to screen contractors requiring escorted access to ATF facilities, unsensitive information, or construction sites for low-risk, nonsensitive duties. The form collects comprehensive personal information from the subject, including residential and employment history for the past 5 years, demographic details, and consent for a background check.

The document outlines a detailed screening process where the contractor completes sections 1-14, providing personal details and demographics. The Contracting Officer's Technical Representative (COTR) or ATF Point of Contact (POC) completes sections 15-24, specifying the ATF office, job title, and access requirements. The Physical Security Programs Branch or Field Division then conducts final checks, including NCIC and TECS checks, and determines compliance with Allied Nations policy and residency requirements. The form emphasizes that a felony or incidents involving firearms or explosives may result in disqualification, and the information provided is used for preliminary screening and identification purposes.
