Instructions The information collected on this form is used to determine your acceptability for Federal and Federal contract employment and your enrollment status in the Government's Life Insurance program. You may be asked to complete this form at any time during the hiring process. Follow instructions that the agency provides. If you are selected, before you are appointed you will be asked to update your responses on this form and on other materials submitted during the application process and then to recertify that your answers are true.
All your answers must be truthful and complete. A false statement on any part of this declaration or attached forms or sheets may be grounds for not hiring you, or for firing you after you begin work. Also, you may be punished by a fine or imprisonment (U.S. Code, title 18, section 1001).
Either type your responses on this form or print clearly in dark ink. If you need additional space, attach letter-size sheets (8.5" X 11").
Include your name, Social Security Number, and item number on each sheet. We recommend that you keep a photocopy of your completed form for your records.
Privacy Act Statement The Office of Personnel Management is authorized to request this information under sections 1302, 3301, 3304, 3328, and 8716 of title 5, U. S. Code. Section 1104 of title 5 allows the Office of Personnel Management to delegate personnel management functions to other Federal agencies. If necessary, and usually in conjunction with another form or forms, this form may be used in conducting an investigation to determine your suitability or your ability to hold a security clearance, and it may be disclosed to authorized officials making similar, subsequent determinations.
Your Social Security Number (SSN) is needed to keep our records accurate, because other people may have the same name and birth date. Public Law 104-134 (April 26, 1996) asks Federal agencies to use this number to help identify individuals in agency records. Giving us your SSN or any other information is voluntary. However, if you do not give us your SSN or any other information requested, we cannot process your application. Incomplete addresses and ZIP Codes may also slow processing.
ROUTINE USES: Any disclosure of this record or information in this record is in accordance with routine uses found in System Notice OPM/GOVT-1, General Personnel Records. This system allows disclosure of information to: training facilities; organizations deciding claims for retirement, insurance, unemployment, or health benefits; officials in litigation or administrative proceedings where the Government is a party; law enforcement agencies concerning a violation of law or regulation; Federal agencies for statistical reports and studies; officials of labor organizations recognized by law in connection with representation of employees; Federal agencies or other sources requesting information for Federal agencies in connection with hiring or retaining, security clearance, security or suitability investigations, classifying jobs, contracting, or issuing licenses, grants, or other benefits; public and private organizations, including news media, which grant or publicize employee recognitions and awards; the Merit Systems Protection Board, the Office of Special Counsel, the Equal Employment Opportunity Commission, the Federal Labor Relations Authority, the National Archives and Records Administration, and Congressional offices in connection with their official functions; prospective non-Federal employers concerning tenure of employment, civil service status, length of service, and the date and nature of action for separation as shown on the SF 50 (or authorized exception) of a specifically identified individual; requesting organizations or individuals concerning the home address and other relevant information on those who might have contracted an illness or been exposed to a health hazard; authorized Federal and non-Federal agencies for use in computer matching; spouses or dependent children asking whether the employee has changed from a self-and-family to a self-only health benefits enrollment; individuals working on a contract, service, grant, cooperative agreement, or job for the Federal government; non-agency members of an agency's performance or other panel; and agency-appointed representatives of employees concerning information issued to the employees about fitness-for-duty or agency-filed disability retirement procedures.
Public Burden Statement Public burden reporting for this collection of information is estimated to vary from 5 to 30 minutes with an average of 15 minutes per response, including time for reviewing instructions, searching existing data sources, gathering the data needed, and completing and reviewing the collection of information. Send comments regarding the burden estimate or any other aspect of the collection of information, including suggestions for reducing this burden, to the U.S. Office of Personnel Management, Reports and Forms Manager (3206-0182), Washington, DC 20415-7900. The OMB number, 3206-0182, is valid. OPM may not collect this information, and you are not required to respond, unless this number is displayed.

Green Mtn Penstock Amendment 0001 Questions and Answers
1) Specification 35 21 96 lists only the 2 Expansion Joints at the Tube Valve to Be Replaced.
Drawing 245-D-60048 Mentions Penstock Expansion Joints not shown. Are we leaving the existing packing in place at those expansion Joints?
A: No. Refer to specification section 35 21 96 Paragraphs 1.08 B. and C. for removal and replacement of the penstock expansion joint packing material and the tube valve wall gland packing material. DWG 245-D-60063, Step 3 of the Cross-Section of Disassembled Expansion Joint indicates to remove existing packing material.
2) Specification 01 35 20 Paragraph 1.03.B.8.f States “Rescue team must be immediately available during all entries into a permit-required confined space”. Does mean the rescue team can have no other duties?
A: No, so long as they are at the site and available if needed.
3) Specification 01 35 30 paragraph 1.04.A.4 States “The individual shall be qualified by virtue of education, training, certification, and experience in which professional status in the construction safety field has been established”. Does this mean the person shall have a College degree in addition to experience, educational certs and training for all the hazards to be acceptable?
A: Yes. A certified Safety Professional (CSP), A degree in Safety and Health, etc.
4) Drawing 245-D-60048 - List of Major Work Items, item “I” states to remove and replace packing material (for the pipe expansion joints shown on 245-D-60063). Item “I” work is shown to be completed at the existing 50” diameter tube valves. Drawing 245-D-60063 – Cross Section of Assembled Expansion Joint states that the inside diameter of the pipe expansion joint is 102”. This dimension does not align with drawing 245-D-60048 which indicates the expansion joints to be 50” diameter by the pipeline it is installed within. Drawing 245-D-60037 Detail 3 also indicates that the expansion joints are near the 50” diameter dimension. Please confirm the diameter of the expansion joints.
A: Refer to 35 21 96 for details on the removal and replacement of the penstock expansion joint packing material and the tube valve wall gland packing material.
Refer to DWG 245-D-1089, the wall gland is approximately 64” diameter. Refer to DWG 245-D-60063, the expansion joint is approximately 102” diameter.
5) The RFP states for bidders to include a Make-or-buy program within Volume I, Business Proposal if it is applicable. Can you please confirm that submission of a Make-or-buy plan by the bidders will only be required if the bidder’s proposed bid amount for the project reaches or exceeds the $15 Million value?
A: Yes, it is only applicable when the estimated value is 15 Million or more.
6) Section L.20 (b) identifies the 3 volumes requested, with Volume III containing the pricing content (page L-16). At the top of page L-20 and M-3 (Factor 4 – Price) it requests that price information be presented in Factor 4 (Volume II – Technical Proposal). However, page L-20
(g) states “no cost or price information shall be included in the technical proposal.” Please confirm if Factor 4 is being omitted from the RFP?
A: Refer to L.20 Page L-16 Pricing is contained within Volume III ONLY.
Refer to L.20 – page L-20 Factor 4. This tells you to submit Schedule B in
Volume III. Factor 4 is NOT being omitted from the RFP.
7) Will the USBR please consider providing a 2-week bid extension to this RFP? Bidders often generate the majority of their questions following a project site visit when the exact site conditions are understood. While the virtual site visit provided by the USBR was certainly appreciated, this project will require much greater analysis of the record documents to fully understand and recognize all of the challenges that working at this project site will present, which might have otherwise been obvious from seeing these locations in person. For that reason, I believe the extra time is required to fully consider the nature of this work and generate additional questions and interaction with the USBR.
7a) Can you please extend the proposal due date by two weeks to allow adequate time to coordinate with subcontractors and suppliers to adequately prepare the proposal?
7b) We are bidding as a subcontractor for the coatings and wanted to see if the project bid date can be moved out a week so the technical proposal/information can be properly prepared and sent to the general contractors for review prior to bid date?
7c) Can the bid due date be extended to provide adequate time for accurate estimating and technical proposal work? Additional time is needed for sourcing materials and subcontracting, etc for this project
A: Yes, Amendment 0001 will extend the due date to August 12, 2021.
8) Can both the project's submission date as well as question period be extended one week? This will allow the contractors to review all of the new information brought forward and asked. It will also allow for any follow up questions or clarifications that could arise prior to bid.
A: Based on proposal due date of August 12, 2021, no further questions will be taken after August 4, 2021.
9) Just wanted to know on the past performance section are you just looking for CPARS or do you want supporting documentation giving a description about the project as well?
A: Refer to L.20 Page L-19. The DUNs number is required for the Prime and Subs.
Government will pull the CPARS.
10) Can an alternate coating system be used other than what is specified in the interior penstock coating of Unit 1 and Unit 2?
A: No. Refer to specification section 09 97 02.
11) What are the average ambient air temperatures in the penstock tunnel? What are the average water temperatures inside the penstock?
A: This will be answered in forthcoming Amendment 0002.
12)Will working days or shutdown durations be extended due to winter conditions? Unit 2 outage starts Nov 1st 2022 and ends May 1st 2023. Half of that outage will be in winter and access and controlling ambient conditions will not be conducive.
13) What type of coating and what is the exterior coating thickness on the penstock pipe in the tunnel?
14) CLIN 8 - 05 50 00 Miscellaneous Metalwork, (ladders, safety climb, grating, etc.) was produced on the previous contract. Will these materials be supplied by the Government? If so, please indicate and include the materials to the Government-Furnished Products specification section 01 64 40.
A: No. Refer to section 01 64 40 for the complete list of Government-Furnished Products.
15) Section L & Section M: Factor 3 - Past Performance, can contractors provide past project experience and contact information for completed projects outside of the federal government? Specifically penstock or water pipeline type projects for power companies and/or municipalities since CPARS is specifically for Federal Government projects.
A: Yes, Past Performance on similar projects outside of the Federal Government may be submitted.
16) Specification section 09 97 02, paragraph 1.03, D: States that field coatings contractors must have a current SSPC-QP1 and SSPC-QP2 certifications, as well as the past performance of not less than 3 jobs removing coal tar enamel coatings from structures of similar size and complexity. Would the USBR considering waiving the SSPC-QP2 certification requirement, if the proposed coatings contractor did meet the SSPC-QP1 certification requirement as well as the record of past performance for similar size and complexity projects?
A: No.
17) Can more detail be provided on electrical capacity (volts/amps) and locations of such available plugs?
A: See Section 01 51 00 – Temporary Utilities. Contractors are required to provide their own power via generators and other distribution equipment.
18) In reference to Spec 099702 Tabulation 1. Coating item “a. …There is approximately 25 and 5 linear feet of 8-inch diameter encased drain-pipe from first bend to sump valve for Unit 1 and 2, respectively; 2 linear feet of 8-inch diameter by-pass pipe per unit; and 14 linear feet, including 4-foot wyes, of 14-inch diameter embedded penstock air vent pipe per unit” … Note, contractor is unaware of how to accurately blast and coat these areas with the proposed vinyl system due to many varying limitations. Does BOR have further information on available equipment and methods to complete this specific work scope?
File details come from the government source that posted it. Updated .
