About this file
This document contains a past performance questionnaire for a federal solicitation. The United States Department of the Interior Bureau of Reclamation is seeking well-trained and capable armed land management and public safety personnel to provide services at Parker Dam, located approximately 25 miles south of Lake Havasu City, Arizona and 14 miles north of Parker, Arizona. The past performance questionnaire requests information about a contractor's performance on previous similar contracts and asks evaluators to rate contractors on factors such as relationship with client, management of subcontractors, overall management and conduct, quality of work, quality control, ability to meet schedules, ability to control costs and provide work at a reasonable price, and compliance with labor standards, safety standards, and completion of projects on time. Evaluators are also asked to provide any cure notices, show cause letters, suspensions of payment, or terminations issued on previous contracts and whether they would award another contract to the firm being evaluated.
The questionnaire requests details of the previous contract such as contract number, type, title, award and completion dates, location, initial and final amounts, and description of work. It also asks for the name, company, address, phone number, email, and position of the evaluator completing the form. Space is provided to select ratings of outstanding, above average, satisfactory, marginal, or unacceptable and include any additional comments for each rating factor. At the end, an overall rating for the firm is requested along with the evaluator's signature and date. Completed questionnaires should be submitted to the Bureau of Reclamation contact.
