About this file
This is a revised forms and provisions document that quoters must complete and return with their quote submission for tree thinning and piling services at Oregon Caves National Monument and Preserve.
The document serves as a template containing the required forms and contractual provisions that quoters must include with their quote submissions. It includes a price schedule table that reflects the corrected total project acreage of 216 acres (revised from the previously stated 224 acres), organized by five work units: Cave Creek Campground Unit, Units 3-5-7-9, Units 1-2-4-6-8, Headquarters Unit, and contingency work. Quoters must complete this attachment as part of their responsive quote and submit it by the extended deadline of 3:00 PM PDT on May 28, 2026. The contract is a firm fixed-price award to a single contractor for a 100% small business set-aside (NAICS code 115310, Support Activities for Forestry, with $11.5M size standard). The period of performance runs from July 16 through September 30, 2026. This revision (Revision 1, dated 5/15/26) corrects acreage figures and incorporates updates regarding partial payment information and work standards including clarifications on wood chipper usage, which is permitted only if equipment remains on roads and chips are deposited off-site at the contractor's expense.
