About this file

This is a Past Performance Questionnaire template used by the National Park Service Denver Service Center for federal contract source selection evaluation.

The questionnaire is completed in two stages: the offeror provides company information and details about a past project in Sections A and B, then transmits the form to a reference contact listed on the project. The reference completes Sections C and D with evaluation ratings and comments, returning the completed questionnaire to the contracting officer by a specified deadline. Section E identifies the receiving contracting officer or contract specialist and their contact information. The document includes a Notice to Contractor prohibiting use of questionnaire information for promotional or endorsement purposes.

Section D contains the evaluation framework with five rating categories—Exceptional, Very Good, Satisfactory, Marginal, and Unsatisfactory—applied across six performance areas: Quality (including workmanship, quality control adherence, and technical submittals); Schedule and Time Management (milestone achievement, responsiveness, submittal timeliness, and issue resolution); Cost Control (handling of site conditions, modification pricing, invoice accuracy, and subcontractor payments); Management (project oversight, subcontractor coordination, safety, location factors, owner relationships, and problem-solving); Small Business Subcontracting and Labor Standards (regulatory compliance, disadvantaged business engagement, and subcontracting plan adherence); and Regulatory Compliance (building and life safety codes). References provide detailed comments supporting ratings, particularly for excellent, marginal, and unsatisfactory performance, and conclude with an overall recommendation for the contractor's suitability for similar contracts.
