This is a Project Experience Questionnaire template used to evaluate contractor qualifications based on relevant past project performance. Offerors must submit responses demonstrating experience relative to Project Experience evaluation subfactors identified in solicitation section L, with submissions limited to the quantity of projects and timeframes specified in the solicitation.
The questionnaire requires offerors to provide referenced project information including project title, location, brief description, and project owner contact details. Contractors must be listed with their role (prime or subcontractor), trade, original and final contract prices, and original and final performance periods in calendar days. Offerors must then address seven substantive questions: describing all project work performed; identifying which Project Experience elements each contractor addressed; explaining price changes with supporting reasons; explaining schedule changes with supporting reasons; describing problems encountered and resolutions implemented, and how the proposal accounts for similar issues; describing successful means and methods and whether they would be implemented on the proposed project with justification; and providing any additional relevant project information. Response sections may be expanded across multiple pages as needed to adequately address each question.
