About this file
This is a Vendor's Participant List form from Broward County related to a federally assisted contract funded by the U.S. Department of Transportation, specifically for opportunities at the Fort Lauderdale-Hollywood International Airport (FLL). The form is designed to collect comprehensive information about prime vendors and their subcontractors, with the goal of establishing and maintaining a database of firms participating in federally assisted transportation contracts. The document requires detailed reporting on business characteristics, including company type, work description, NAICS codes, contract amounts, annual gross receipts, and demographic information about the firm's majority owner.
The form is part of the Disadvantaged Business Enterprise (DBE) or Airport Concession Disadvantage Business Enterprise (ACDBE) Program requirements, which mandate specific documentation for federal contract participation. Vendors must categorize themselves as DBE or Non-DBE, and provide detailed information about their business, including federal tax ID, firm name, contact information, year established, and contract-specific details. The form includes multiple sections for prime vendors (Part A) and subcontractors/subconsultants (Part B), with checkboxes for business type, annual revenue ranges, and owner demographics. Failure to complete and submit the form within the specified timeframe may result in the vendor being deemed non-responsive to the solicitation.
