About this file
This is a certification attachment document required by SUNY College of Technology at Alfred for the Integrated Student Health & Counseling Electronic Health Record System procurement in New York. Vendors must submit proposals for an integrated EHR system supporting student health, counseling, psychiatry, and wellness services while maintaining FERPA and HIPAA compliance. The procurement allows for contract terms of 3, 4, or 5 years. Questions regarding the solicitation must be submitted no later than August 10, 2026, with responses provided by August 14, 2026. Proposals must be submitted electronically via email to the designated campus contact. The procurement is subject to NYS Finance Law Sections 139-j and 139-k (Lobbying Law), and vendors are restricted to contacting only the designated individual(s) listed in the advertisement to avoid potential violations resulting in contract rejection or debarment from state sales.
By submitting a bid, each bidder certifies under penalty of perjury that it has implemented a written policy addressing gender-based violence in the workplace meeting the requirements of New York Executive Law Section 575, subdivision 11, and has provided this policy to all employees, directors, and board members. Bidders unable to make this certification must submit a signed statement detailing reasons for non-certification; failure to provide either the certification or explanatory statement will result in bid rejection. The successful contractor must submit vendor responsibility documents, comply with SFL 139-j and 139-k requirements, provide NYS Tax Law Section 5-a documentation, and submit valid current NYS Workers' Compensation and Disability forms. The award amount is up to $125,000 for the system procurement.
