About this file
This is a Quality Assurance Surveillance Plan (QASP) for copier/printer preventative maintenance and repair services at Glen Canyon Field Division, covering a base year plus four option years.
The QASP establishes systematic monitoring methods to ensure contractor compliance with the Performance Work Statement (PWS). Government oversight is managed by the Contracting Officer (CO), who maintains overall contract administration and authority; the Contracting Officer's Representative (COR), who serves as the primary monitor for contractor performance; and Government Technical Points of Contact (POCs) at specific copier locations. Key performance objectives monitored include quarterly preventative maintenance, repair services, response times, equipment uptime, loaner equipment provision, and service reporting. The contractor must complete preventative maintenance every three months with 100% compliance and zero missed visits. Repair services must restore full device functionality using OEM components, with 95% of repairs completed without issue recurrence within 30 days. Critical repairs require on-site response within one business day (95% compliance), while routine repairs must be completed within three business days (95% compliance). The contractor must contact the POC within 60 minutes of service call placement (95% compliance). Equipment uptime must meet 95% per machine monthly, excluding approved downtimes. Loaner equipment must be delivered within two business days once the five-business-day repair threshold is reached, with 100% compliance required. Service reports must be submitted after every visit with 100% compliance. Surveillance methods include 100% inspection for PM compliance and loaner deadlines, random sampling for repair quality, customer feedback from POC reports, contractor self-reporting, and escort log verification. The COR maintains comprehensive documentation of all service reports, repair logs, uptime calculations, notification timestamps, escort logs, and corrective actions to support annual option-year evaluations and contractor performance reports. If performance falls below acceptable quality levels, the CO may require corrective action plans, reduce invoice payments, suspend payment approval, decline to exercise option years, issue cure notices, or terminate the contract for cause.
