1.01 ADDENDUM

The Prescott National Forest is seeking bids from qualified contractors to perform road maintenance and/or construction of the following Roads: FSR-261, 70, 81, 82 & 52. This will consist of repairing soil erosion damaged roads and eliminating surface protruding rocks. Drainage control of surface water with proper drain features. This will include:

cleaning bar ditches, lead-outs and rolling dips. Install new bar ditches, lead-outs and rolling dips where needed. Clearing brush, clean out two cattle guards of debris and clean out any culverts if any. Contractor and COR will determine best locations for installing new bar ditches, lead-outs and rolling dips.

This project consists of but not limited to, providing all materials, equipment, labor, supervision and any assistance necessary for the construction and maintenance services at the following site locations:

Prescott National Forest Service Roads:

FSR-261 (Approx. 0.7 Mi. - See Location Map) FSR-81 (Approx. 0.45 Mi. - See Location Map) FSR-70 (Approx. 0.6 Mi. - See Location Map) FSR-82 (Approx. 0.75 Mi. - See Location Map) FSR-52 (Approx. 3.8 Mi. - See Location Map)

Phase One - FSR-52 (Approx. 3.8 Mi.) Involves Heavy road maintenance and repair.

Heavy UTV & 4x4 traffic along with drainage issues have caused soil erosion and damage to the road. This has caused ruts and potholes throughout the road and have exposed protruding rocks that require to be eliminated from the road surface. The road bar diches are completely filled and require cleaning. There are two culverts (contractor to field verify) and two cattle guards that require cleaning as well. You will need to install new lead-outs and rolling dips. Contractor and COR will determine best locations for installing new lead-outs and rolling dips.

Phase Two - FSR-261 (Approx. 0.7 Mi.) Involves Heavy road maintenance and repair.

Requires road to be a minimum of 12’ in width (where needed), clear brush, repairing soil erosion road damage, eliminate surface protruding rocks, install bar ditches, clean one cattleguard, install new lead-outs and rolling dips. Contractor and COR will determine best locations for installing new bar ditches, lead-outs and rolling dips. (See Location Map)

Phase Three - FSR-82 (Approx. 0.75 Mi.) Involves Heavy road maintenance and repair.

Requires the road to be a 12’ minimum width, clear brush and install new bar ditches, lead-outs and rolling dips. Contractor and COR will determine best locations for installing new bar ditches, lead-outs and rolling dips.

Phase Four - FSR-81 (Approx. 0.45 Mi.) Requires repairing soil erosion road damage, road with to be 12’ minimum width, install new bar ditches, clear brush and install new lead-outs and rolling dips. Contractor and COR will determine best locations for installing new lead-outs and rolling dips.

Phase Five - FSR-70 (Approx. 0.6 Mi.) requires the construction of a new road with a 12’ minimum width, clear brush and install new lead-outs and rolling dips. Contractor and COR will determine best locations for installing new lead-outs and rolling dips.

It is the intention of the Government to award all Phases at the same time; however, if the entire bid exceeds available funding, then the Government, at its discretion, may award only phase one or award phases one and two, awarding phases three thru five at a later time. It is advantageous to the Contractor to offer a discount if phase one thru five are awarded and performed at the same time.
