Hazardous Substance Containing

Materials Summary

1.0 Asbestos Inspection Methodology

The asbestos survey was conducted to supplement a previous inspection report conducted in 1992. The previous report excluded some interior materials and the exterior building materials.

Homogeneous areas were identified and suspect materials within each area were categorized as surfacing, thermal system insulation, or miscellaneous. Homogeneous materials are those building materials that, by visual and manual inspection, are similar in texture, color, composition, and use in the building. The condition of each suspect material was assessed in the field by the inspector.

The condition of the materials was classified as good, damaged, or significantly damaged. A Sampling Location Key Plan is included as Attachment A for building and area sample location reference numbers for the survey. Each structure/area section of this report contains respective sample location maps for the structure/area sampled.

Federal and state regulatory agencies define ACM as any building material that contains greater than 1% asbestos. The presence and percentage of asbestos can only be determined by laboratory analysis. Asbestos samples were sent to EMSL Analytical, Inc. (EMSL) in Houston, Texas. EMSL participates in the National Institutes for Standards and Testing (NIST) National Voluntary Laboratory Accreditation Program (NVLAP) proficiency testing program (NVLAP No. 101587-0).

Samples were analyzed by Polarized Light Microscopy (PLM) by EPA method 600/R/R-93/116. All percentages reported for composition are based on visual estimation or gravimetric determinations. Laboratory reports are provided with each building and area sampled throughout the report. Asbestos containing materials summary is provided in Table 1-1 below. An ACM Quantity Summary is provided as Attachment A of this Report.

Regulation 21 as revised by the Commission as of January 2014 defines Regulated Asbestos- Containing Material (RACM) as:

(A) Friable asbestos material;

(B) Category I non-friable ACM that has become friable;

(C) Category I non-friable ACM that will be or has been subjected to sanding, grinding, cutting, or abrading;

(D) Category II non-friable ACM that has a high probability of becoming or has become crumbled, pulverized, or reduced to powder by the forces expected to act on the material in the course of demolition or renovation operations regulated by this regulation;

(E) Category I non-friable resilient floor covering which contains ACM that will be or has been removed by sanding, grinding, cutting, or abrading; or

(F) Category II mastic which contains ACM that will be removed by sanding, grinding, cutting, or abrading.)

Harbor recommends following ADEQ regulations which require notice of demolition of these structures. A project design will be required, along with appropriately certified workers and contracting company. A notice of intent (NOI), along with a copy of the asbestos survey, is required to be filed with ADEQ a minimum of 10 days prior to beginning work.

2.0 PCB Inspection Methodology

The PCB portion of the survey consisted of inspecting the capacitors in the light ballasts in the buildings proposed for demolition. Capacitors in which the label did not state “No PCBs” are assumed to contain PCBs and are listed in Table 2-1 below.

Table 2-1 PCBs Building 5: Healthcare

Description and Location Qty

Old fluorescent light capacitors throughout the building with two (2) ballasts, observed on the walls, were found to contain PCBs. 14

Pole-mounted transformer south of Carport 1

According to 40 Code of Federal Regulations (CFR) § 761.60(b)(2) any person may dispose of PCB small capacitors as municipal solid waste.

Harbor recommends following disposal methods for PCB transformers according 40 CFR 760.60(b)(1)(i)(A) or (B). Option A includes disposing of transformer in an incinerator in compliance with 40 CFR761.70. Option B includes disposing of the transformer in a chemical waste landfill approved under 40 CFR 761.75; provided that all free-flowing liquid is removed from the transformer. See regulations for more detail disposal information.

3.0 Lead-based Paint Sampling

The lead inspection procedures consisted of collection of discrete paint samples of visible paint material in the planned renovation area. Positive lead-based paint sample results are provided in Table 3-1 below. Federal and state regulatory agencies define a lead-based paint as containing 0.5% lead by weight.

Table 3-1 Positive Lead-based Paints

CPP-1 exceeded the Federal Agencies definition of a lead-based paint at 0.5% with a result of 1.23%. The brown/green paint on the carport overhang was peeling at the time of the site visit.

There are no regulations that require removal of LBP prior to renovation activities. However, OSHA does require that workers be protected from potential exposure to lead during the project.

Therefore, Harbor recommends that the Forest Service inform the renovation contractors of the lead-based paint discovered.

EMSL maintains liability limited to cost of analysis . Interpretation and use of test results are the responsibility of the client. This report relates only to the samples reported above, and may not be reproduced, except in full, without written approval by EMSL. EMSL bears no responsibility for sample collection activities or analytical method limitations . The report reflects the samples as received.

Results are generated from the field sampling data (sampling volumes and areas, locations, etc.) provided by the client on the Chain of Custody. Samples are within quality control criteria and met method specifications unless otherwise noted. The above analyses were performed in general compliance with Appendix E to Subpart E of 40 CFR (previously EPA 600/M4-82-020 “Interim Method”) but augmented with procedures outlined in the 1993 (”final”) version of the method. This report must not be used by the client to claim product certification, approval, or endorsement by NVLAP, NIST or any agency of the federal government. Non- friable organically bound materials present a problem matrix and therefore EMSL recommends gravimetric reduction prior to analysis . Unless requested by the client, building materials manufactured with multiple layers (i.e. linoleum, wallboard, etc.) are reported as a single sample. Estimation of uncertainty is available on request.

Samples analyzed by EMSL Analytical, Inc. Houston, TX NVLAP Lab Code 102106-0, AZ 0925, CO AL-15355, LA 04126, TX 300159

Building 5: Healthcare

Building 5: Healthcare See Map HA2 for Sample Locations

The Healthcare building is located just south/southwest of the gate on the west side of the Job Corp facility. The building is constructed on a concrete slab with wood-framed walls and a stucco exterior with a pitched, asphalt-shingled roof. The building consists of dental offices, exam rooms, restrooms, a waiting area, bed wards, a storage closet, dark room, and medical lab rooms. Building 5 is approximately 3,480 square feet. The building was inspected for suspect ACM and LBP, and other potentially hazardous materials. The sampled/observed materials and corresponding laboratory results, if applicable, are provided in Table 5-1, 5-2, and 5-3 below. Photographs of sampled/potentially hazardous materials from the 2021 HazMat Survey are included in Attachment A. Laboratory results are presented as Attachment B. This table is supplemented with sample results from the 1992 Asbestos Evaluation Report prepared by Asbestos Abatement Services, Inc.
