About this file
This is a Request for Quotation (RFQ) template document for the procurement of commercial supplies under FAR Part 12 streamlined procedures. The template is designed to combine synopsis and solicitation functions and can be used for firm fixed-price stand-alone contracts or IDIQ contracts utilizing simplified acquisition procedures or negotiation processes. The document serves as an instruction guide for contracting officers, containing extensive pre-populated FAR clauses, provisions, and representations and certifications sections that must be tailored by individual agencies based on their specific procurement needs.
The related federal contract opportunity (Solicitation 12444126Q0019) applies this template to a specific requirement: the Department of Agriculture Forest Service is seeking a Dozer transport trailer for the Apalachicola National Forest Wakulla Ranger District to support fire program area management. This is a Total Small Business Set-Aside with a NAICS code of 336212 and a size standard of $7,500,000. Offerors must submit three separate attachments (Technical Proposal, Price Proposal, and Representations and Certifications) via email to thomas.wolterman@usda.gov by March 16, 2026 at 1700 ET, with questions due by 1500 ET the same day. Delivery is requested as soon as possible to Wakulla WC, 10703 Bloxham Cutoff Road, Tallahassee, FL 32305. The evaluation will consider technical capability, past performance, and price, with award to the responsible offeror whose offer is most advantageous to the government.

Template Name: NRE_Commercial Supplies_Combined SS_RFP or RFQ Updated through FAC 2025-04 effective June 11, 2025, FAR Part 1 Model Deviation, FAR Part 10 Model Deviation, and FAR Part 34 Model Deviation.
Template Use: To solicit and award a firm fixed price (FFP) stand-alone contract or IDIQ contract for commercial supplies that utilize FAR subpart 12.6 Streamlined Procedures for Evaluation and Solicitation for Commercial Items procedures in conjunction with FAR part 13 Simplified Acquisition Procedures OR with FAR part 15 Contracting by Negotiation procedures. This combines the synopsis with the solicitation, so if a synopsis is required (ie: > $25K unless an exception applies) and you don't wish to synopsize, this template may be used to eliminate the synopsis requirement. (See FAR subparts 5.2 and FAR subpart 12.6). Applicable FAR clauses and provisions shall be selected by the user based on current FAR prescriptions. Clauses and provisions without comments should remain in all contracts/solicitations. Delete clauses and provisions that don’t apply to your action. This document may not be all-inclusive for every situation. The Contracting Officer is responsible to ensure all clauses and provisions applicable to each project are included.
To assist with choosing sustainable acquisition clauses, see Sustainable Acquisition Clause Checklists Oct 23.docx (sharepoint.com). Solicitation Contents: Generate a solicitation in IAS. Upload all solicitation documents into the IAS solicitation and obtain necessary reviews prior to releasing. The SF-1449 isn’t used as part of your solicitation in a combined synopsis/solicitation advertisement, but IAS makes you choose a form, so choose that one. The attached sections can be posted directly to the GPE along with any attachments needed (ie: tech specifications).
Award: Utilize the SF-1449 for awards over SAT. The SF-1449 is non-mandatory but encouraged for awards under SAT. For the body of the contract: add the vendor’s price schedule and remove the provisions (they are labeled as Provisions so it’s easier to identify what can be removed) and reps and certs. Generate the award from the solicitation in IAS, upload all pertinent award documents and obtain required reviews prior to releasing.

For simplified acquisitions, the word quote or quoter is substituted in provision FAR 52.212-1 for the word offer or offeror.
Addenda to Provision FAR 52.212-1 paragraph (b) Submission of Offers:
1) Offerors must have an active entity registration in the System for Award Management to submit an offer. https://www.sam.gov/SAM/

In the event that there is no past performance information available, the offeror will receive a neutral rating in this factor. |
| ii) Technical Capability – Provide a list of equipment and key personnel (with qualifications) that will be used to complete this project. |

4) Address questions about this solicitation to ___Thomas Wolterman____________ at ___thomas.wolterman@usda.gov____________.
Addenda to Provision FAR 52.212-1 paragraph (f) Late submissions: Comment by Johnson, Melissa -FS [2]: Include verbiage like this if you want to allow the acceptance of late offers.
Replace (f)(2)(i) with the following:
Any offer, modification, revision, or withdrawal of an offer received at the Government office designated in the solicitation after the exact time specified for receipt of offers is "late" and will not be considered unless it is received before award is made and the Contracting Officer determines that accepting the late offer would not unduly delay the acquisition. Delete FAR 52.212-1 (f)(2)(i) paragraphs A, B, & C.
(End of provision)
(ix) FAR 52.212-2 Evaluation – Commercial Products and Commercial Services (NOV 2021) (Provision) Comment by Johnson, Melissa -FS: When the use of evaluation factors is appropriate, the contracting officer may (1) Insert the provision at 52.212-2, Evaluation-Commercial Items, in solicitations for commercial items (see 12.602); or
(2) Include a similar provision containing all evaluation factors required by 13.106, subpart 14.2 or subpart 15.3, as an addendum (see 12.302(d)).
FILL IN THE BLANKS. Coordinate with provision 52.212-1 addenda so that you are requesting sufficient documentation to address the factors here. Include a biobased factor when applicable. CO must justify the use of the LPTA source selection process. Not to be used for IT, telecom, professional services, PPE, or training. (see FAR 13.106-1(a)(2)(v) and FAR 15.101-2)
(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be the lowest price technically acceptablemost advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:
[Contracting Officer shall insert the significant evaluation factors, such as (i)technical capability of the item offered to meet the Government requirement; (ii)price; (iii)past performance (see FAR 15.304); and include them in the relative order of importance of the evaluation factors, such as in descending order of importance.]
| Technical and past performance, when combined, are __________ [Contracting Officer state, in accordance with FAR 15.304, the relative importance of all other evaluation factors, when combined, when compared to price.] | Comment by Johnson, Melissa -FS [2]: You can remove this for Part 13 requirements if you don't wish to state the importance of the factors in comparison to price. |
| (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). | Comment by Johnson, Melissa -FS: If you don’t have options, this paragraph may be removed. Re-letter paragraph c. There are other option clauses that could be used so make sure this doesn’t contradict one of those. |
(bc) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer’s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
