About this file

This document is an attachment to a federal janitorial services contract for the Ely Ranger District Office (Contract 1240LS26Q0054) that specifies contractor credentialing and training requirements.

The document establishes a USDA Credential Matrix outlining IT and physical access rights based on employee classification and duration of assignment. Access types include Privileged IT Access, General IT Access, Privileged Physical Access, and General Physical Access, each with specific authorization levels and approval requirements (checkmarks indicate required credentials like UnCPass, PIV-A, Site Badge, or No Badge). The mandatory contractor training section applies to new service contracts awarded after November 1, 2023, and modified service contracts as applicable. Contractor employees and subcontractor personnel must complete five annual training courses through AgLearn (or paper-based for USDA Records Management): Understanding and Interrupting Unconscious Bias (90 minutes, due within 90 days of assignment); Anti-Harassment Training (60 minutes, due within 90 days); Information Security Awareness (60 minutes, due within 1 year); USDA Records Management (60 minutes, due within 60 days); and Section 508 (60 minutes, due within 45 days). Contractors must maintain completion records and provide them to the Contracting Officer Representative upon request.

Background checks compliant with Presidential Directive HSPD-12 are required within ten calendar days of contract award for each new employee, with employees providing name, address, social security number, date of birth, and work phone to the Contracting Officer Representative in person. The government covers up to three background investigations within a 12-month period; excessive personnel turnover (more than three replacements in a rolling 12-month period) results in contractor-funded investigations, with costs deducted from the final invoice.
