About this file

This is a Fire Plan form template used by the USDA Forest Service to establish fire prevention and suppression protocols for contractors performing work on national forest lands. The form designates responsible parties for fire management, including the contractor's representative, the Contracting Officer's Representative, and Forest Service inspectors, with contact information for each. The contractor is required to take reasonable and practical action to prevent and suppress fires in the project area and must take suppression action immediately upon discovering a fire, while the Forest Service retains the right to supplement contractor efforts or assume control of suppression operations if reinforcements are needed.

The form establishes fire prevention requirements including designation of named fireguard(s) responsible for fire detection and suppression, specification of whether employee smoking and open fires are permitted in the project area (both subject to written authorization by the Contracting Officer's Representative), and identification of Forest Service employees to whom fires must be reported. The contractor must identify employees with fire qualifications and their best fire assignments, document available firefighting equipment and vehicles by description and quantity, and acknowledge joint preparation of the fire plan through signatures from both the contractor (or designated representative) and the Contracting Officer's Representative.
