About this file
This document is a compliance checklist template for evaluating offeror self-performance and subcontracting disclosure in accordance with FAR 52.219-14, Limitations on Subcontracting.
The checklist requires offerors to complete five sections in full, with non-responses or incomplete responses potentially rendering the quote ineligible for award. Section 1 establishes the performance approach, requiring offerors to indicate whether the prime contractor will self-perform the required work or subcontract a portion. Section 2 requires offerors intending to subcontract to confirm subcontracting plans. Section 3 mandates detailed information for each projected subcontractor, including legal business name, Unique Entity ID (UEI), projected NAICS code for subcontracted work, and certification that the subcontractor is listed as a small business under the applicable NAICS in SAM.gov; subcontractors not certified as small businesses are ineligible for use under small business set-asides and must be replaced or removed. Section 4 requires prime contractor compliance certification, with the authorized official certifying that the prime will not subcontract more than the allowable percentage to non-similarly situated firms, that all projected subcontractors are verified small business concerns in SAM.gov, and that the prime understands the consequences of non-compliance with FAR 52.219-14, including potential termination for default, negative CPARS ratings, or other government remedies. The document includes signature blocks for the authorized official's certification and an optional Section 5 for clarifying remarks regarding performance approach or subcontracting strategy.
