About this file

This is an Offeror Self-Performance & Subcontracting Disclosure Checklist required for compliance with FAR 52.219-14, Limitations on Subcontracting. The checklist must be completed in full by all offerors responding to Solicitation 1232SA26Q0887, a Total Small Business Set-Aside for BSL-2 + Lab 247 Floor Replacement services under NAICS Code 238330 with a $19M size standard. Nonresponses or incomplete responses may render a quotation ineligible for award.

The checklist requires offerors to disclose whether the prime contractor will self-perform the required work or utilize subcontractors. If subcontracting is planned, offerors must identify each projected subcontractor's legal business name, Unique Entity ID (UEI), and projected NAICS code, and certify that all subcontractors are verified small business concerns in SAM.gov. The prime contractor must certify compliance with the 50 percent subcontracting limitation, ensuring that no more than 50 percent of government-paid contract performance is subcontracted to firms that are not similarly situated, with any further subcontracting by similarly situated entities counting toward this threshold. An authorized official must sign and date the completed checklist. The solicitation is issued by the Department of Agriculture Agricultural Research Service as a Request for Quotation with multiple amendments posted.
