AMD 0001 ATTACHMENT 001 PERFORMANCE WORK STATEMENT AND QASP

Laboratory Sterilizer Maintenance Service

1. General

1.1. Background: The Center for Grain and Animal Health (CGAHR) Research is seeking a company with extensive knowledge and competence calibrating temperature and pressure sensor on Getinge sterilizers and have Getinge autoclave passwords. Downtime of the sterilizers during calibration services shall be kept to a minimum. The facility will be occupied during this work. The CGAHR campus address is 1515 College, Manhattan, KS.

1.2. Scope : The contractor shall calibrate temperature and pressure sensors for sterilizers located at CGAHR. Additionally, the contractor shall provide documentation (calibration certificate, supporting records, and measurement uncertainties) that the equipment being used to calibrate the sensors have been calibrated within one year. The contractor shall submit reports of the calibration work.

2. Contractor Performance Requirements:

2.1. Conduct preventative maintenance travel and labor once within the base period and once per any awarded option periods.

2.2. Repair travel and labor.

2.3. Telephone response within 4 hours.

2.4. Onsite response 48 (business hours).

2.5. Provide calibration reports, including supporting records and measurement uncertainties, for all serviced equipment.

2.6. Building will be occupied while service is ongoing. Contractor shall schedule work with location to allow location to meet current scientific and research needs.

2.7. Contractor’s Key Personnel: The contractor shall provide a repair service technician that is authorized by the OEM manufacturer of the serial numbered equipment detailed in Section 1.3 above to perform and document the service requirements.

2.8. Security Requirements: All contractor personnel shall always be in the presence of a

USDA escort while on location.

3. Quality Assurance Surveillance Plan (QASP): The Government shall establish a QASP as a means of evaluating whether the Contractor is meeting the performance standards/quality levels identified in the PWS to ensure that the government pays only for the level of services received.
