2.0 Asbestos Survey
Thunderbird completed an asbestos survey of the Site structures on March 19, 2018. The survey was performed by Martin Brogie, LEP, licensed as an Asbestos Inspector by the Connecticut Department of
Public Health (Certificate Number 275).
The survey consisted of a National Emissions Standards for Hazardous Air Pollutants (NESHAP) assessment that included a thorough inspection of the subject building components/areas in which the location and number of samples collected, using limited destructive testing, provided sufficient detail to estimate the quantity and location of ACM located in the study areas. The federal NESHAP sampling protocols (40 CFR
61.145), utilized for the asbestos assessment of structures, equipment and buildings to be demolished, requires the following:
“…the owner or operator of a demolition or renovation activity and prior to the commencement of the demolition or renovation, thoroughly inspect the affected facility or part of the facility where the demolition or renovation operation will occur for the presence of asbestos…”
The intent of these federal regulations is to require suspect ACM which will be disturbed during the demolition or renovation (including repair), be assessed for asbestos content (defined as greater than 1% asbestos) and, if identified, be removed and disposed of properly. The asbestos survey utilized both statistically random and discrete sampling procedures during the assessment to find safely accessible sampling locations as well as ensuring that as many suspect building materials were sampled as possible.
Samples were collected using a utility knife and other hand tools which were decontaminated in between samples to avoid cross-contamination. Collected samples were placed in plastic sample bags, labeled with a unique identification number, documented in field notes and on the chain-of-custody documents. Upon completion of sampling activities, field renderings of the survey areas were created which displayed the sample locations. Samples were shipped under chain of custody to International Asbestos Testing
Laboratories, Inc. in Mount Laurel New Jersey for asbestos analysis (Polarized Light Microscopy).
Sampling locations are shown in the attached site building sketches, included as Attachment A. Copies of the laboratory analytical reports are provided as Attachment B. The table below summarizes the sampled materials and the asbestos analysis results.

Notes:
1. ND - No asbestos detected
2. L.F. – linear feet
3. s.f - square feet
4. P.C. – Asbestos Content by Point Count Method
Asbestos was detected in various roofing materials on the detached garage building and the residence including vent flashing and an asphalt/tar roofing layer on the main roof of the residence. Asbestos tar flashing was also found at the junction of the front porch roof and house. White caulking located on the underside of the wooden roof soffit (behind the vinyl cladding) was found to contain asbestos. The caulking was only observed on the south side of the main roof and was applied to gaps in the wood sheathing line at the house trim.
The first floor bathroom contains asbestos flooring and an asbestos plaster wall coating. The identified first floor bathroom flooring material is located below newer flooring layers and plywood. The first floor bathroom plaster application containing asbestos is textured, and applied to plaster (walls and ceiling) that does not have a ceramic tile application. Asbestos-containing white fabric insulating materials were
ATTACHMENT 28
Hazardous Materials Survey Report 69 3rd Ave Extension, West Haven, Connecticut identified on wall-mounted hot air ducts including two in the living room and one in each of the three second floor bedrooms. The insulation materials extend along the ductwork from the vents in each room and down through the walls and kitchen ceiling to the basement ceiling duct system. No such material was noted on basement ductwork, which appears to have been replaced. The second floor bathroom contains asbestos floor tile and is placed on a wood floor.
The estimated ACM quantity for the Site is 72 linear feet and 2,040 square feet.
Asbestos abatement will be necessary prior to building demolition.
3.0 Lead-Based Paint Testing
Lead-based paint is often found on windows, trim, baseboards and exterior components of older homes.
As part of the hazardous materials survey conducted on March 19, 2018, interior and exterior paint chip samples were collected and submitted to a CT DPH certified laboratory for analysis of lead by EPA Method
6010C. The interior sample contained lead at 73 mg/kg and the exterior sample contained lead at 90,600 mg/kg.
The exterior paint sample was collected from the original siding and from front porch trim. The concentration detected is not uncommon for exterior surfaces on homes built prior to 1960. Based on the volume of exterior paint relative to the overall volume of the residence, it is unlikely that a whole building composite sample, for disposal characterization, will exceed the 5 mg/L (analyzed via the Toxicity
Characteristic Leaching Procedure) threshold rendering the waste stream hazardous for lead. However, a representative waste stream sample must be collected and analyzed to make this demonstration.
4.0 Aboveground Storage Tanks (ASTs) and Underground Storage Tanks (USTs) According to the City of West Haven Tax Assessor’s database, the Site building is heated with natural gas;
however, a 275-gallon heating oil AST was observed in the basement. The AST was noted to be approximately ¼-full.
5.0 Mercury and PCB Containing Materials
As part of the Site inspection on March 19, 2018, Thunderbird observed one thermostat within the Site building. The thermostat has the potential to contain minimal amounts of liquid mercury. Two mercury containing light bulbs were observed on the south wall of the 1st floor living room.
Two PCB-containing ballasts were observed along the south wall of the 1st floor living room.
6.0 Pressure Treated Wood
No pressure-treated wood was observed at the Site.
ATTACHMENT 28
Hazardous Materials Survey Report 69 3rd Ave Extension, West Haven, Connecticut
7.0 Mold
Thunderbird did not observe any evidence of mold at the Site during the inspection conducted on March
19, 2018.
8.0 Other Potentially Hazardous Material
A ½-full gallon can of latex paint was observed in the basement. A ¾-full gallon can of plaster roof cement and a ½-full gallon can of exterior enamel paint were observed in the garage.
9.0 Conclusions and Recommendations
Thunderbird Environmental, LLC (Thunderbird) has performed a pre-demolition hazardous materials assessment for the property located at 69 3rd Avenue Extension in West Haven, Connecticut. The assessment included an asbestos survey, lead-based paint sampling, and an evaluation of the presence of other miscellaneous hazardous/regulated materials that may require special disposal or handling prior to, or during demolition.
Based on the findings of the survey, asbestos abatement is required prior to building demolition.
Miscellaneous household/maintenance chemicals and paints should be removed from the residence and disposed/recycled prior to demolition. The amount of household/maintenance materials requiring removal is not anticipated to exceed 100 kilograms. The oil AST will require draining, cleaning and disposal.
The potential mercury containing thermostat should be disposed of in accordance with Connecticut Public
Act 12-54. The mercury containing light bulbs should be recycled prior to demolition.
High lead concentrations in exterior paint will need to be sampled as part of a whole building analysis for lead to confirm waste disposal requirements.
10.0 Limitations
Suspected ACM observed during the hazardous materials survey were sampled and analyzed for asbestos.
While our inspections are comprehensive, there may be limited areas, such as areas between walls and beneath structures, which were inaccessible and therefore not inspected or tested.
All estimates of materials provided are based on our observations made at the time of our inspection and should be confirmed by abatement contractors.

Information Pertinent to this Report:
Analysis by US EPA 600 93-116: Determination of Asbestos in Bulk Building Materials by Polarized Light Microscopy (PLM).
Certifications:
NIST-NVLAP No. 101165-0• NYSDOH-ELAP No. 11021• AIHA-LAP, LLC No. 100188•
Quantification at <0.25% by volume is possible with this method. (PC) Indicates Stratified Point Count Method performed. (PC-Trace) means that asbestos was detected but is not quantifiable under the Point Counting regimen. Analysis includes all distinct separable layers in accordance with EPA 600 Method. If not reported or otherwise noted, layer is either not present or the client has specifically requested that it not be analyzed (ex. analyze until positive instructions). Small asbestos fibers may be missed by PLM due to resolution limitations of the optical microscope. Therefore, PLM is not consistently reliable in detecting asbestos in non-friable organically bound (NOB) materials. Quantitative transmission electron microscopy (TEM) is currently the only method that can pronounce materials as non-asbestos containing.
Analytical Methodology Alternatives: Your initial request for analysis may not have accounted for recent advances in regulatory requirements or advances in technology that are routinely used in similar situations for other qualified projects. You may have the option to explore additional analysis for further information. Below are a few options, listed as the matrix followed by the appropriate methodology. Also included are links to more information on our website.
Bulk Building Materials that are Non-Friable Organically Bound (NOB) by Gravimetric Reduction techniques employing PLM and TEM: ELAP 198.6 (PLM-NOB), ELAP 198.4 (TEM-NOB)
Loose Fill Vermiculite Insulation, Attic Insulation, Zonolite (copyright), etc.: US EPA 600 R-4/004 (multi-tiered analytical process) Sprayed On Insulation/Fireproofing with Vermiculite (SOF-V): ELAP 198.8 (PLM-SOF-V)

Disclaimers / Qualifiers:
There may be some samples in this project that have a "NOTE:" associated with a sample result. We use added disclaimers or qualifiers to inform the client about something that requires further explanation. Here is a list with highlighted disclaimers that may be pertinent to this project. For a full explanation of these and other disclaimers, please inquire at customerservice@iatl.com.
1) Note: No mastic provided for analysis.
2) Note: Insufficient mastic provided for analysis.
3) Note: Insufficient material provided for analysis.
4) Note: Insufficient sample provided for QC reanalysis.
5) Note: Different material than indicated on Sample Log / Description.
6) Note: Sample not submitted.
7) Note: Attached to asbestos containing material.
8) Note: Received wet.
9) Note: Possible surface contamination.
10) Note: Not building material. 1% threshold may not apply.
11) Note: Recommend TEM-NOB analysis as per EPA recommendations.
12) Note: Asbestos detected but not quantifiable.
13) Note: Multiple identical samples submitted, only one analyzed.
14) Note: Analyzed by EPA 600/R-93/116. Point Counting detection limit at 0.080%.
15) Note: Analyzed by EPA 600/R-93/116. Point Counting detection limit at 0.125%.
16) Note: This sample contains >10% vermiculite mineral. See Appendix for Recommendations for Vermiculite Analysis.
Recommendations for Vermiculite Analysis:
Several analytical protocols exist for the analysis of asbestos in vermiculite. These analytical approaches vary depending upon the nature of the vermiculite mineral being tested (e.g. un-processed gange, homogeneous exfoliated books of mica, or mixed mineral composites). Please contact your client representative for pricing and turnaround time options available.
iATL recommends initial testing using the EPA 600/R-93/116 method. This method is specifically designed for the analysis of asbestos in bulk building materials. It provides an acceptable starting point for primary screening of vermiculite for possible asbestos.
Results from this testing may be inconclusive. EPA suggests proceeding to a multi-tiered analysis involving wet separation techniques in conjunction with PLM and TEM gravimetric analysis (EPA 600/R-04/004).
For New York State customers, NYSDOH requires disclaimers and qualifiers for various vermiculite containing samples that direct analysis via ELAP198.6 and ELAP198.8 for samples that contain >10% vermiculite mineral where ELAP198.6 may be used to evaluate the asbestos content of the material. However, any test result using ELAP198.6 will be reported with the following disclaimer: “ELAP198.6 method does not remove vermiculite and may underestimate the level of asbestos present in a sample containing >10% vermiculite.”
Further information on this method and other vermiculite and asbestos issues can be found at the following: Agency for Toxic Substances and Disease Registry (ATSDR) www.atsdr.cdc.gov, United States Geological Survey (USGS) www.minerals.usgs.gov/minerals/, US EPA www.epa.gov/asbestos. The USEPA also has an informative brochure "Current Best Practices for Vermiculite Attic Insulation" EPA 747F03001 May 2003, that may assist the health and remediation professional.
The following is a summary of the analytical process outlines in the EPA 600/R-04/004 Method:
