USDA-NRCS Connecticut State Office 344 Merrow Road, Suite A Tolland, Connecticut 06084

Attn: Kristin Walker, Civil Engineer

RE: Hazardous Materials Survey Report 44 Blohm Street, West Haven, Connecticut

Dear Ms. Walker:

Thunderbird Environmental, LLC (Thunderbird) is pleased to submit this hazardous materials survey report to the Natural Resources Conservation Service (NRCS) for the above referenced property located at 44

Blohm Street in West Haven, Connecticut (herein after referred to as “the Site”). This report presents the findings of our property inspection and summarizes the observed presence of potentially hazardous/regulated substances including: asbestos containing materials (ACM), lead-based paint, heating oil tanks, mercury and polychlorinated biphenyl (PCB) containing materials, pressure treated wood, mold, and other miscellaneous hazardous materials (paint, automotive fluids, household cleaners, etc.). In addition, Thunderbird contracted laboratory analysis of potential ACM and lead-based paint. This report also provides estimated quantities of hazardous/regulated substances, but does not quantify removal/disposal costs.

1.0 Site Description

The Site is comprised of an approximately 0.11-acre residential property located on the southeast side of

Blohm Street in West Haven, Connecticut (Figure 1). The site includes an approximately 3,091 square-foot, three-story, multi-family residence with an unfinished basement and a detached shed. According to the City of West Haven Tax Assessor’s database, the building was constructed in 1930.

The residence is constructed with wood framing on a mortared concrete block and stone foundation. The exterior is clad with vinyl siding over painted wood siding. The basement is accessed via a bulkhead door on the rear (southeast side) of the building. The basement contained three gas-fired boilers and miscellaneous maintenance-related containers as further described in this report. Apartments are located on each of the three floors above the unfinished basement and are accessed via front and rear stairways.

The apartments each include three bedrooms, one bathroom, and a kitchen, living room and dining room.

The third-floor apartment included a ceiling hatch accessing a fiberglass-insulated attic. The attic provided access to the roof via a hinged roof hatch.

All of the apartments had been renovated including the removal of former plaster walls and the replacement of walls and ceiling with sheetrock. Newer electrical and heating systems were also noted.

Hardwood floors were located in all rooms except the kitchen, bathroom and hallways. Tile or linoleum had been placed on to the wooden floors in these areas. The roof consisted of one-layer of asphalt shingles.

A wooden, two-bay shed is located to the rear of the residence. It is primarily constructed of scrap wood including pallets and re-used window sashes. Some roofing materials are also located on a portion of the shed.

A wooden fence is located along the rear portion of the northeast property boundary.

2.0 Asbestos Survey

Thunderbird completed an asbestos survey of the Site structures on March 22, 2018. The survey was performed by Martin Brogie, LEP, licensed as an Asbestos Inspector by the Connecticut Department of

Public Health (Certificate Number 275).

The survey consisted of a National Emissions Standards for Hazardous Air Pollutants (NESHAP) assessment that included a thorough inspection of the subject building components/areas in which the location and number of samples collected, using limited destructive testing, provided sufficient detail to estimate the quantity and location of ACM located in the study areas. The federal NESHAP sampling protocols (40 CFR

61.145), utilized for the asbestos assessment of structures, equipment and buildings to be demolished, requires the following:

“…the owner or operator of a demolition or renovation activity and prior to the commencement of the demolition or renovation, thoroughly inspect the affected facility or part of the facility where the demolition or renovation operation will occur for the presence of asbestos…”

The intent of these federal regulations is to require suspect ACM which will be disturbed during the demolition or renovation (including repair), be assessed for asbestos content (defined as greater than 1% asbestos) and, if identified, be removed and disposed of properly. The asbestos survey utilized both statistically random and discrete sampling procedures during the assessment to find safely accessible sampling locations as well as ensuring that as many suspect building materials were sampled as possible.

ATTACHMENT 27

Hazardous Materials Survey Report 44 Blohm Street, West Haven, Connecticut

Samples were collected using a utility knife and other hand tools which were decontaminated in between samples to avoid cross-contamination. Collected samples were placed in plastic sample bags, labeled with a unique identification number, documented in field notes and on the chain-of-custody documents. Upon completion of sampling activities, field renderings of the survey areas were created which displayed the sample locations. Samples were shipped under chain of custody to International Asbestos Testing

Laboratories, Inc. in Mount Laurel New Jersey for asbestos analysis (Polarized Light Microscopy).

Sampling locations are shown in the attached site building sketches, included as Attachment A. Copies of the laboratory analytical reports are provided as Attachment B. The table below summarizes the sampled materials and the asbestos analysis results.

Asbestos was detected in five (5) materials sampled throughout the residence including: chimney flue cement; grey backing under flooring in the 2nd floor front hallway; linoleum (two types) in the 2nd floor kitchen and hallway; and, black mastic associated with the roof tar paper.

The estimated ACM quantity for the Site is 1,481 square feet.

Asbestos abatement will be necessary prior to building demolition.

3.0 Lead-Based Paint Testing

Lead-based paint is often found on windows, trim, baseboards and exterior components of older homes.

As part of the hazardous materials survey conducted on March 22, 2018, interior and exterior paint chip samples were collected and submitted to a CT DPH certified laboratory for analysis of lead by EPA Method

6010C. The interior sample contained lead at 2.96 mg/kg and the exterior sample contained lead at

18,500 mg/kg.

The exterior paint sample was collected from the original siding and from the front porch trim. The concentration detected is not uncommon for exterior surfaces on homes built prior to 1950. Based on the volume of exterior paint relative to the overall volume of the residence, it is unlikely that a whole building composite sample, for disposal characterization, will exceed the 5 mg/L (analyzed via the Toxicity

Characteristic Leaching Procedure) threshold rendering the waste stream hazardous for lead. However, a representative waste stream sample must be collected and analyzed to make this demonstration.

4.0 Aboveground Storage Tanks (ASTs) and Underground Storage Tanks (USTs)

According to the City of West Haven Tax Assessor’s database, the Site building is heated with natural gas.

Three gas-fired boilers were observed in the basement. No fill or vent pipes were observed along the exterior of the Site building.

ATTACHMENT 27

Hazardous Materials Survey Report 44 Blohm Street, West Haven, Connecticut

Approximately 6-inches of a ½-inch diameter copper line was observed adjacent to one of the gas-fired boilers. There was no other evidence of current or former ASTs and USTs at the Site. It is possible that the residence was historically served by one or more oil tanks.

5.0 Mercury and PCB Containing Materials

As part of the Site inspection on March 22, 2018, Thunderbird observed three thermostats within the Site building. The thermostats have the potential to contain minimal amounts of liquid mercury. There was no other evidence of potential mercury containing material at the Site.

Thunderbird did not observe any potential PCB containing material at the Site.

6.0 Pressure Treated Wood

A pressure-treated wooden fence was observed located along the rear half (approximately) of the eastern property boundary.

7.0 Mold

Several panels of sheetrock were observed to be destroyed on the 1st and 2nd floor of the Site building form an apparent burst pipe. In addition to the observed water damage, black-colored mold was noted on much of the sheetrock located in the residence.

8.0 Other Potentially Hazardous Material

Several cans of latex paint were observed in the basement. The majority of paint cans were noted to be less than ½-full.

Miscellaneous household repair materials including: joint compound, paint thinner, pourable sealer, and tile adhesive were observed in the basement.

9.0 Conclusions and Recommendations

Thunderbird Environmental, LLC (Thunderbird) has performed a pre-demolition hazardous materials assessment for the property located at 44 Blohm Street in West Haven, Connecticut. The assessment included an asbestos survey, lead-based paint sampling, and an evaluation of the presence of other miscellaneous hazardous/regulated materials that may require special disposal or handling prior to, or during demolition.

Based on the findings of the survey, asbestos abatement is required prior to building demolition.

Miscellaneous household/maintenance chemicals and paints should be removed from the residence and disposed and/or recycled prior to demolition. The amount of household/maintenance materials requiring removal is not anticipated to exceed 100 kilograms.

High lead concentrations in exterior paint will need to be sampled as part of a whole building analysis for lead to confirm waste disposal requirements.

ATTACHMENT 27

Hazardous Materials Survey Report 44 Blohm Street, West Haven, Connecticut

In accordance with Connecticut Department of Energy and Environmental Protection (DEEP) guidelines, disposal of the pressure treated wood should be handled as follows:

• Contractors, utilities, and manufactures should contract directly with a DEEP permitted bulky waste landfill, or send it to an out-of-state wood burner facility.

The three thermostats should be disposed of in accordance with Connecticut Public Act 12-54.

Dust control measures should be implemented when handling mold-containing material. The presence of mold on sheetrock would not be expected to effect disposal requirements.

10.0 Limitations

Suspected ACM observed during the hazardous materials survey were sampled and analyzed for asbestos.

While our inspections are comprehensive, there may be limited areas, such as areas between walls and beneath structures, which were inaccessible and therefore not inspected or tested.

All estimates of materials provided are based on our observations made at the time of our inspection and should be confirmed by abatement contractors.

We appreciate the opportunity to assist NRCS with this project. If you have any questions or comments, please contact us at 860-227-4714.

Appendix to Analytical Report Customer Contact: Rory Parcell Method: US EPA 600, R93-116

This appendix seeks to promote greater understanding of any observations, exceptions, special instructions, or circumstances that the laboratory needs to communicate to the client concerning the above samples. The information below is used to help promote your ability to make the most informed decisions for you and your customers.

Please note the following points of contact for any questions you may have.

General Terms, Warrants, Limits, Qualifiers:

General information about iATL capabilities and client/laboratory relationships and responsibilities are spelled out in iATL policies that are listed at www.iATL.com and in our Quality Assurance Manual per ISO 17025 standard requirements. The information therein is a representation of iATL definitions and policies for turnaround times, sample submittal, collection media, blank definitions, quantification issues and limit of detection, analytical methods and procedures, sub-contracting policies, results reporting options, fees, terms, and discounts, confidentiality, sample archival and disposal, and data interpretation.

iATL warrants the test results to be of a precision normal for the type and methodology employed for each sample submitted. iATL disclaims any other warrants, expressed or implied, including warranty of fitness for a particular purpose and warranty of merchantability. iATL accepts no legal responsibility for the purpose for which the client uses test results. This confidential report relates only to those item(s) tested and does not represent an endorsement by NIST-NVLAP, AIHA LAP LLC, or any agency of local, state or province governments nor of any agency of the U.S. government.

This report shall not be reproduced except in full, without written approval of the laboratory.

Information Pertinent to this Report:

Analysis by US EPA 600 93-116: Determination of Asbestos in Bulk Building Materials by Polarized Light Microscopy (PLM).

Certifications:

NIST-NVLAP No. 101165-0• NYSDOH-ELAP No. 11021• AIHA-LAP, LLC No. 100188•

Quantification at <0.25% by volume is possible with this method. (PC) Indicates Stratified Point Count Method performed. (PC-Trace) means that asbestos was detected but is not quantifiable under the Point Counting regimen. Analysis includes all distinct separable layers in accordance with EPA 600 Method. If not reported or otherwise noted, layer is either not present or the client has specifically requested that it not be analyzed (ex. analyze until positive instructions). Small asbestos fibers may be missed by PLM due to resolution limitations of the optical microscope. Therefore, PLM is not consistently reliable in detecting asbestos in non-friable organically bound (NOB) materials. Quantitative transmission electron microscopy (TEM) is currently the only method that can pronounce materials as non-asbestos containing.

Analytical Methodology Alternatives: Your initial request for analysis may not have accounted for recent advances in regulatory requirements or advances in technology that are routinely used in similar situations for other qualified projects. You may have the option to explore additional analysis for further information. Below are a few options, listed as the matrix followed by the appropriate methodology. Also included are links to more information on our website.

Bulk Building Materials that are Non-Friable Organically Bound (NOB) by Gravimetric Reduction techniques employing PLM and TEM: ELAP 198.6 (PLM-NOB), ELAP 198.4 (TEM-NOB)

Loose Fill Vermiculite Insulation, Attic Insulation, Zonolite (copyright), etc.: US EPA 600 R-4/004 (multi-tiered analytical process) Sprayed On Insulation/Fireproofing with Vermiculite (SOF-V): ELAP 198.8 (PLM-SOF-V)

Recommendations for Vermiculite Analysis:

Several analytical protocols exist for the analysis of asbestos in vermiculite. These analytical approaches vary depending upon the nature of the vermiculite mineral being tested (e.g. un-processed gange, homogeneous exfoliated books of mica, or mixed mineral composites). Please contact your client representative for pricing and turnaround time options available.

iATL recommends initial testing using the EPA 600/R-93/116 method. This method is specifically designed for the analysis of asbestos in bulk building materials. It provides an acceptable starting point for primary screening of vermiculite for possible asbestos.

Results from this testing may be inconclusive. EPA suggests proceeding to a multi-tiered analysis involving wet separation techniques in conjunction with PLM and TEM gravimetric analysis (EPA 600/R-04/004).

For New York State customers, NYSDOH requires disclaimers and qualifiers for various vermiculite containing samples that direct analysis via ELAP198.6 and ELAP198.8 for samples that contain >10% vermiculite mineral where ELAP198.6 may be used to evaluate the asbestos content of the material. However, any test result using ELAP198.6 will be reported with the following disclaimer: “ELAP198.6 method does not remove vermiculite and may underestimate the level of asbestos present in a sample containing >10% vermiculite.”

Further information on this method and other vermiculite and asbestos issues can be found at the following: Agency for Toxic Substances and Disease Registry (ATSDR) www.atsdr.cdc.gov, United States Geological Survey (USGS) www.minerals.usgs.gov/minerals/, US EPA www.epa.gov/asbestos. The USEPA also has an informative brochure "Current Best Practices for Vermiculite Attic Insulation" EPA 747F03001 May 2003, that may assist the health and remediation professional.

The following is a summary of the analytical process outlines in the EPA 600/R-04/004 Method:

1)Analytical Step/Method: Initial Screening by PLM, EPA 600R-93/116 Requirements/Comments: Minimum of 0.1 g of sample. ~0.25% LOQ for most samples.

2)Analytical Step/Method:Wet Separation by PLM Gravimetric Technique, EPA R-04/004 Requirements/Comments: Minimum 50g** of dry sample.
