14. DESCRIPTION OF AMENDMENT/MODIFICATION (Organized by UCF section headings, including solicitation/contract subject matter where feasible.)
The purpose of this amendment is to accomplish the following:
1. Provide answers to industry submitted questions during the second RFI period, revise "SPECIFICATIONS", section (d)(4), "Packing and Labeling¿, extend the RFQ due date, and update "ITEMS 2 KIT CONTENT LIST". See content of following pages for details.
Except as provided herein, all terms and conditions of the document referenced in Item 9 A or 10A, as heretofore changed, remains unchanged and in full force and effect .

The purpose of this amendment is to provide answers to industry submitted questions during the RFI period, revise “SPECIFICATIONS”, section (d)(4) “Packing and Labeling”, extend the RFQ due date, and update “ITEMS 2 KIT CONTENT LIST”. No additional questions will be answered unless deemed in the best interest of the Government by the Contracting Officer.
ANSWERS TO INDUSTRY SUBMITTED QUESTIONS:
QUESTION 1 (multi-part):
“We went in and looked at the packing list and there are a few discrepancies form the list we now fill.
Item 2.4 has 32 boxes of 100 count 1 x 3 elastic bandages that is 3200 bandages and will take up a good portion of the box.(this is a huge cost and would be exceedingly wasteful) 2 boxes is the norm.
Item 2.34 has disposable heating pads instead of cold packs.
Item 2.37 has 36 bt of hand sanitizer we have only ever put in 6
Item 2.56 Ranitidine was replaced with Famotidine after a recall for Ranitidine a few years ago.
We have searched for boxes and there are none available in the size requested. Where do you order yours so we can contact them?”
ANSWER:
- Item 2.4: See updated packing list for “NFES #001760 Kit, First Aid, Medical Support”.
- Item 2.34: See updated packing list for “NFES #001760 Kit, First Aid, Medical Support”.
- Item 2.56: No change, see “SECTION REVISION” below concerning substitutions.
- The US Forest Service get the specified boxes from Defense Logistics Agency (DLA) NSN# 8115001390673. DLA does not list a vendor.
SECTION REVISION:
“SPECIFICATIONS”, section (d)(4) “Packing and Labeling” is changed to state:
Only those products and services specifically listed in this BPA are authorized. Unauthorized supplies or services without Contracting Officer approval shall be at the risk of and the sole responsibility of the Vendor and may result in non-payment as well as cancellation of this BPA. If an item required in the Kit Content Lists is not available at the time of BPA Call Order, the vendor may submit a request to the COR to supply an alternate item(s) at the individual Call Order level if the item listed in the Kit Content Lists for NFES #001143 & NFES #001760 are not available at the time of order based on supply chain availability and lead times. Any alternate item shall be of “equal to or greater than” in every capacity to the item listed in the Kit Content List. The vendor shall not include any alternate item without written approval of the Contracting Officer.
If an item required in the Kit Content List is not available at the time of a BPA Call Order, a “greater than or equal” cannot be provided due to market availability, and/or the lead time for the item or substitution is greater than the BPA Call Order due date, the vendor may request the partial order be delivered to the US Forest Service location, minus the unobtainable item.
The vendor shall not ship any partial BPA Call Order without the approval of the Contracting Officer.
REVISED REQUEST FOR QUOTE DUE DATE:
The RFQ due date is changed FROM 5PM CT, Monday, 8/8/2022, TO 5PM CT, Monday 8/22/2022 to accommodate changes addressed in this modification.
