Past Performance: Submit a list of relevant projects completed within the last five (5) years, of the date proposals are due, that are similar in scope and complexity to the project identified in this solicitation. For each project, submit the project name, location, description of the work, dollar value and name/phone/email of project owner representative.

Evaluation Procedures, 1. Past Performance in its entirety with the following:

Evaluation (Section M): Past performance is a measure of the degree to which the offeror

(including major subcontractors identified in the offeror's proposal) has satisfied previous customers and complied with Federal, State and

Local laws and regulations. The Government will contact references to assess the quality of the contractor's work, including history of quality workmanship, timely completion, reasonable and cooperative behavior, concern for customer satisfaction and compliance with contract requirements. The Government will consider the currency and relevance of the information, sources of the information, context of the data and general trends in the contractor performance.

The past performance evaluation will characterize the level of the Government's confidence that the offeror will successfully complete the work, in compliance with all contract requirements, based on its record of successful performance on recent and relevant similar projects.

In the past performance evaluation, the

Government may use information from sources other than the offeror's proposal. Other sources of information, may include, but are not limited to:

Federal, State and Local government agencies, better business bureaus, published media and electronic databases such as the Contractor

Performance Assessment Reporting System (CPARS).

Where there is little or no information available regarding the offeror's past performance history because the offeror is new to the marketplace, the past performance evaluation may take into account information regarding predecessor companies, key personnel who have relevant experience, or subcontractors that will perform major or critical aspects of the work. When no information is available, the offeror will not be evaluated favorably or unfavorably on past performance.

Emailed Question: Clarification request on

Kitchen sink model SOW pg. 16 12.3 indicates a

Elkay DLR332210 and Delta 400-WFELHHDF faucet

A-02 Equipment Schedule indicates Kohler

K-RH5267-4-NA for both the "accessible" and

"remaining" units. What does the Forest Service want?

Forest Service Response: The kitchen sinks will be the Kohler K-RH5267-4-NA in the non-ABA units.

The SOW is for for the non-ABA units. The ABA equipment identified on A-02 (Washer, Dryer, Oven, Dishwasher) are all ABA compliant. The kitchen sink (Kohler K-RH5267-4-NA) is not ABA compliant. The Forest Service did not directly specify the equipment required for the ABA unit in terms of toilet, shower/tub, bathroom sink, kitchen sink. The Forest Service provided the

ABA requirements and it is up to the contractor to provide equipment that meets ABA requirements.

Additionally, the Forest Service is not replacing appliances in the non-ABA units except for the one (1) dishwasher in Unit 14.

Emailed Question: Clarify number of toilets

Forest Service response: Only three (3) toilets are required. The toilet in Unit 14 (2 Bedroom, upstairs, non-ABA) was replaced last year.

All other terms and conditions remain unchanged.
