Vendor Questionnaire for Interior Signage Contract - Broward County, Florida
This is a Vendor Questionnaire form for Broward County Public Safety Building 2nd Floor interior signage procurement in Florida. The contract scope requires the manufacture, packaging, and delivery of all signage identified in the contract documents, with signs delivered complete, labeled, and ready for installation. Installation will be performed by Broward County. The successful bidder must coordinate delivery dates with the Broward County Project Manager and ensure all signs are packaged to prevent transportation and storage damage. Any damaged or defective signage upon delivery shall be replaced by the manufacturer at no additional cost to Broward County. All deliveries must occur during normal business hours, Monday through Friday, unless otherwise approved by the County. Vendors must submit the completed questionnaire with their solicitation response or within three business days of written County request; failure to timely submit may result in nonresponsiveness determination.
The questionnaire requires vendors to provide standard business information including legal name, FEIN, office location, contact details, and authorized representatives. Vendors must certify they have not been debarred or suspended by any government entity within three years, are currently authorized by the manufacturer as a dealer with manufacturer's warranty coverage, and will extend the same pricing and terms to other governmental entities during the contract period. Vendors must confirm acceptance of Visa credit card payment without additional fees, complete non-collusion certification, disclose any participation in solicitation development, and optionally certify implementation of a drug-free workplace program for potential award preference in cases of tied submittals. All certifications require signature by an authorized vendor representative acknowledging that inaccurate statements may result in rejection, award rescission, contract termination, or vendor debarment.
