About this file

This is a reference questionnaire document issued by the Nevada State Purchasing Division, Department of Administration, for solicitation number 10TCA-S3688. The State of Nevada requires vendors and subcontractors to submit client references for projects of similar size and scope as part of the formal solicitation process. The completed reference questionnaires must be submitted via email to the single point of contact, Annette Morfin (amorfin@admin.nv.gov), by the due date of April 8, 2026, with the solicitation number included in the subject line. The questionnaire is designed to be completed by clients or former clients of the proposing vendor and must address all evaluation items, with respondents instructed to answer "U/K" (unknown) or "N/A" (not applicable) where appropriate.

The reference questionnaire requires clients to rate vendor performance across twelve key criteria using a 1-10 scale, with detailed performance benchmarks provided (0 for poor/inadequate; 1-3 for below average; 4-6 for average; 7-9 for above average; and 10 for exceptional). Evaluation areas include vendor knowledge and expertise, flexibility with scope and timeline changes, quality of delivered materials and products, staff dynamics, schedule adherence, customer service responsiveness, billing accuracy, problem resolution capability, and likelihood of future recommendations. Additional evaluation items request qualitative feedback on the vendor's most satisfactory service aspects and overall willingness to recommend the vendor. Completed reference questionnaires are designated as confidential business information under Nevada Revised Statutes 333.020(5)(b) and are not subject to public disclosure, ensuring candid client feedback.
