About this file
This document is a Foreign Country of Concern Attestation (Form PUR 1355) required by the state of Florida for entities submitting bids, proposals, or replies to, or entering into, renewing, or extending a contract with a Governmental Entity which would grant the entity access to an individual's Personal Identifying Information. The attestation requires the entity to declare that it is not owned by, organized under the laws of, or have its Principal Place of Business in a Foreign Country of Concern, and that the government of a Foreign Country of Concern does not have a Controlling Interest in the entity.
The related state and local contract opportunity is a Request for Proposals (RFP) issued by the Florida Department of Juvenile Justice, Office of Prevention Services, for community organizations, faith-based groups, non-profits, LLCs, and local government entities to develop and implement comprehensive programs focused on preventing juvenile delinquency among youth aged 10 to 17 throughout Florida. The RFP seeks the implementation and delivery of innovative programming to address juvenile delinquency prevention efforts through evidence-based, promising practices, or demonstrated effectiveness that support mentoring services in high delinquency areas and low performing school districts.
