About this file
This document is a PWC100 and DSA Box Upload Information Form issued by the California Department of Industrial Relations (DIR) in accordance with Senate Bill 854, establishing requirements for public works contractor registration and compliance. The form serves as a data collection instrument for all contractors and subcontractors bidding on or performing work on public works projects in California, including the Culinary Arts Modernization Project at William S. Hart Union High School District. The form requires submission of comprehensive contractor information including company name, contact details, CSLB and DIR registration numbers, project identification, and classification of work by trade (such as carpenters, electricians, laborers, and other skilled trades). A mandatory job walk is scheduled for October 15, 2025, at 10:00 am at Golden Valley High School, with electronic bid submissions due via the District's eProcurement Portal by November 3, 2025, at 2:00 pm. The project spans two phases: Phase 1 from December 22, 2025, to January 9, 2026, and Phase 2 from March 25, 2026, to June 30, 2026, across six Santa Clarita high schools.
All public works contractors must fulfill four key responsibilities: register as a public works contractor with annual renewal (at costs of $400, $800, or $1,200 for one, two, or three fiscal years respectively), pay prevailing wages, follow apprenticeship requirements, and maintain and submit certified payroll records to DIR. The project requires a single prime contractor with a Class B General Contractor license and mandates compliance with prevailing wage laws, extensive insurance requirements under the Statewide Educational Wrap Up Program (SEWUP) with approximately $800 million in total liability coverage, a Workers' Compensation Experience Modification Rate of 1.25 or less, and zero Serious and Willful violations in the past five years. The scope includes removal and replacement of countertops, sinks, and electrical infrastructure across six high schools, with a $50,000 owner-controlled allowance, and the project emphasizes encouraging participation from Disabled Veterans Business Enterprises (DVBE).
