About this file
This document is an Infection Control Risk Assessment (ICRA) for a construction project to update wayfinding signage on the Hines campus of the Edward Hines Jr. VA Hospital. The project, with number 578-15-008, will be managed by Jake Kanuru and is scheduled to run from October 1, 2023, to December 24, 2024.
The ICRA classifies the construction activities into four types (A-D) and four risk groups (1-4) based on the level of dust, noise, and vibration. It outlines the required actions for the contractor based on the determined project class, which ranges from I to IV. These actions include measures to minimize dust, contain waste, isolate work areas, maintain negative air pressure, and perform cleaning and disinfection at various stages of the project. The document requires signatures from the contractor, project planner, supervisor, and infection control personnel to authorize the work.

CLASS
I
1. Execute work to minimize air-borne dust dispersed into atmosphere from construction operations.
2. Immediately replace any ceiling tile displaced for visual inspection.
3. Contain construction waste before transport in tightly-covered containers.
4. Clean work area upon completion of task including wiping surfaces with disinfectant.
5. Contact EMS for appropriate preparation for return to operation(s).
CLASS
II
In addition to above:
6. Post the signed ICRA at the entrance.
7. Isolate work area from patients/staff. Use appropriate temporary barriers and more permanent containment methods (i.e., seal holes in walls, tape gaps in doors, etc.)
8. Water mist work surfaces to control dust while cutting.

11. Wet mop and/or vacuum w/HEPA filtered vacuum before leaving work areas.
CLASS
III
In addition to above:
12. Install barriers to create a negative pressure ante room and/or workroom.
13. Maintain negative air pressure in work area at all times until completion utilizing HEPA-equipped air filtration units.
14. Vacuum personnel with HEPA vacuum before leaving. Or, Don-Doff coveralls each time leaving work site. All wear shoe covers changed each time exiting the area.
15. Vacuum the entire work area with HEPA vacuums or wet mop with disinfectant at the completion of project.
