About this file

This is a Form 1295 Acknowledgment of Exemptions form issued by Fort Bend Independent School District (FBISD) in Texas, required under House Bill 1295 and Texas Government Code Section 2252.908. The form relates to a contract opportunity for digital radios, airtime services, equipment, and related items for the District's two-way radio system supporting its school bus fleet. The form itself does not specify product quantities, response dates, due dates, site visits, bidder meetings, award dates, or contract terms, as it functions primarily as a compliance document rather than a procurement specification document.

The form requires vendors to certify their status and disclose interested parties as a condition of contract execution with FBISD. Vendors may claim exemption from the disclosure requirement if they fall into specific categories, including sponsored research contracts of higher education institutions, interagency contracts, certain health and human services contracts with undetermined value, contracts with publicly traded business entities or their wholly owned subsidiaries, or contracts with electric or gas utilities. Vendors must check the applicable exemption box, sign the certification, and submit the completed form through the Bonfire procurement system. The document emphasizes that failure to comply with the disclosure requirements may result in contract delays or termination.
