About this file

This is a Lead Inspection/Risk Assessment Report Checklist issued by the Michigan Department of Health and Human Services (MDHHS) in May 2018 (Version 3) that establishes compliance standards for certified Lead Inspectors and Risk Assessors conducting environmental investigations on residential properties. The checklist outlines mandatory report components and documentation requirements aligned with Michigan's Act, corresponding Rules, ASTM standards, and guidance from the U.S. Department of Housing and Urban Development (HUD), the Environmental Protection Agency (EPA), and MDHHS. The checklist is not exhaustive, and MDHHS reserves the right to modify it without prior notice. Reports must be completed and delivered within 20 business days after completion of the lead inspection and/or risk assessment, with copies distributed to both the building owner and the contracting party.

The checklist establishes detailed requirements across multiple report sections including title pages with property and inspector information; lead testing documentation with positive paint results, dust wipe samples, soil samples, and surfaces unable to be tested; inspector certification details; and five comprehensive appendices covering resident interviews, site information with floor and site plans, educational materials on lead hazards and federal disclosure rules, XRF device results with performance characteristic sheets, and laboratory reports from NLLAP-recognized facilities. Additional requirements include chain of custody documentation for samples, a glossary of lead-related terms, written disclosure of any affiliations between inspectors and abatement contractors, and notification of lead identification activities to authorities by the 15th of the following month. This checklist applies to the City of Detroit Office of Contracting and Procurement's three-year Environmental Investigation Firms contract for providing environmental services at multiple Detroit locations.

Lead Inspection / Risk Assessment

Report Checklist

This checklist is not intended to be an all-inclusive list. Complying with this list does not guarantee that a certified Lead Inspector/Risk Assessor will not receive a violation as a result of an enforcement inspection. Inspections will utilize Michigan’s Act, corresponding Rules, ASTM standards and interpretive guidance from the U.S. Department of Housing of Urban

Development (HUD), the Environmental Protection Agency (EPA), and the Michigan Department of Health and Human Services (MDHHS) to determine compliance. MDHHS reserves the right to modify this compliance checklist without prior notice.
