About this file
This is a State Corporation Commission Requirement Form from the Commonwealth of Virginia related to a Grease Trap Pumping and Cleaning contract for the Fairfax County Public School System (PPS). The document establishes compliance requirements under Virginia Code Section 2.2-4311.2 and mandates that all bidders provide documentation of their authorization to transact business in Virginia. Bidders must identify their State Corporation Commission registration number if they are organized as a corporation, limited liability company, business trust, limited partnership, or registered limited liability partnership. Alternatively, bidders may certify that they are not required to register, provide documentation of exemption as an out-of-state entity, or submit a legal opinion explaining why their Virginia contacts do not constitute transacting business in the Commonwealth. The form allows for a waiver option for bidders with pending SCC applications.
The contract requires ongoing compliance with Virginia registration requirements throughout the contract term, with provisions allowing the public body to void the contract if a bidder fails to maintain required business entity authorization or allows their registration to lapse. Failure to provide the required SCC identification information or qualifying statement may result in bid rejection unless a waiver is granted by the Director of the Department of General Services or the chief executive of the relevant local governing body. No specific pricing terms, contract duration, award dates, or other financial details are provided in this compliance document.
