About this file

This document is a collection of certification forms for the Madison County Transit Invitation for Bid (IFB), Volume 3 of 5, which includes multiple federal compliance certifications. The forms cover requirements for federal tax liability, recent felony convictions, lobbying activities, compliance with the John S. McCain National Defense Authorization Act, and Buy America procurement regulations. Vendors must complete these certifications for contracts exceeding $10,000, with each form requiring detailed attestations about the company's compliance with specific federal regulations.

The certifications mandate that vendors confirm they have no unpaid federal tax liability, have not been convicted of federal felonies in the past 24 months, have not used federal funds for lobbying purposes, do not utilize prohibited telecommunications equipment, and comply with Buy America requirements for steel, iron, and manufactured products. Failure to accurately complete these certifications can result in significant penalties, including civil penalties up to $100,000 and potential disqualification from the bidding process. The forms are designed to ensure transparency, legal compliance, and ethical business practices for federal and federally-assisted procurements.
