About this file
This is a Drug Free Workplace Form for Hillsborough County Board of County Commissioners in Florida related to the Ambulance Transport Billing and Collection Services contract opportunity. The form establishes compliance requirements for contractors bidding on or executing contracts with the County. It outlines the County's commitment to maintaining a drug-free workplace and requires contractors to certify their adherence to applicable federal drug-free workplace regulations and state laws. The document serves as a mandatory certification document that bidders must complete and submit as part of their proposal package for the five-year ambulance transport billing and collection services contract valued at $9,360,000.00 for the Hillsborough County Fire Rescue Department.
The form requires contractors to make specific certifications regarding drug-free workplace compliance, including statements about unlawful drug manufacture, distribution, possession, or use in the workplace. Contractors must acknowledge their understanding of the consequences of non-compliance and commit to maintaining a drug-free environment. Execution of this certification form is a prerequisite for contract award, and false certification may result in contract termination or debarment from future County contracting opportunities. This standard compliance document reflects the County's conditions of doing business and is a common requirement for state and local government contracts in Florida.
