About this file
This document is a Drug-Free Workplace Affidavit required for the Letcher County Fiscal Court's invitation for bid on demolition services in Kentucky. The affidavit establishes mandatory workplace compliance requirements that the responding firm must implement as a condition of contract award. The contractor must publish a statement prohibiting unlawful manufacture, distribution, dispensing, possession, or use of controlled substances in the workplace and specifying consequences for violations. Additionally, the firm must inform all employees about drug abuse dangers, the company's drug-free workplace policy, available counseling and rehabilitation programs, and penalties for violations. Each employee engaged in providing the commodities or contractual services under bid must receive a copy of the drug-free workplace statement and acknowledge agreement to abide by its terms.
The affidavit further requires that employees notify their employer within five days of any conviction or guilty plea related to controlled substance violations occurring in the workplace. Any employee convicted of a drug-related offense must be subject to sanctions or required to participate satisfactorily in a drug abuse assistance or rehabilitation program if available in the community. The responding firm must make a good faith effort to maintain ongoing compliance with these drug-free workplace requirements throughout the contract term. This affidavit serves as a certification document that contractors must execute and submit with their bids for the Letcher County demolition services project.
