About this file

The document is a Drug-Free Workplace Affidavit that outlines specific requirements for a responding firm regarding workplace drug policies. The affidavit requires the firm to take six key steps to establish and maintain a drug-free workplace, including publishing a statement prohibiting controlled substance activities, informing employees about drug abuse dangers, providing employees with a drug-free workplace statement, and establishing notification procedures for drug-related convictions.

The affidavit mandates that the firm implement comprehensive measures to address drug use, such as giving employees a copy of the drug-free workplace statement, requiring employees to notify the employer within five days of any drug-related conviction, and imposing sanctions or requiring participation in drug rehabilitation programs for employees convicted of drug offenses. The document serves as a certification that the responding firm is committed to maintaining a safe and drug-free work environment as a condition of bidding on or performing contractual services.
