1.0 DESCRIPTION OF SERVICES
1.1. Organizations. 1 SOCES/CEIOB
1.2. Scope of Work. The Hurlburt Field Advanced Wastewater Treatment Facility (HFAWWTF) operates under the regulatory purview of the Department of Environmental Regulation (FDEP), as well as, the United States Environmental Protection Agency (USEPA). The operation is defined by an operational permit that specifies limitations and monitoring requirements of surface water discharge and reclaimed water. The testing and analysis provides data to satisfy this compliance requirement. A cost effective approach of conducting these tests and analysis is a competitive bid service contract.
Contractor shall furnish all labor, materials, supplies, and transportation to accomplish the required testing and analysis. The testing and analysis work to be performed includes the following principle features:
1.2.1. Contractor shall be National Environmental Laboratory Accreditation Program (NELAP) certified.
1.2.2. Contractor shall deliver sample containers prior to sampling dates.
Contractor shall pick up samples and/or perform testing and analysis in accordance with paragraph 1.3.
1.2.3. Samples shall be picked up at the Hurlburt Field Advanced Wastewater Treatment Facility (Building 91500). Sampling days will typically be Monday, Tuesday, Wednesday, and Friday. In the event Monday is a Federal Holiday, sampling for Monday will be changed to Thursday to allow for base access.
Sampling days may be temporarily changed due to extenuating circumstances by either party. Sample collection time not specified will be prearranged between the contractor and HFAWWTF representative.
1.2.4. Sample pick up shall allow sufficient time for sample analysis to ensure test method hold times are not exceeded.
1.2.5. Daily and weekly samples will be collected and prepared for pick up by personnel from HFAWWTF. Chain of Custody (COC) Forms will be prepared by personnel from HFAWWTF.
1.2.5.1. Analysis results and completed COC shall be emailed or hand delivered to HFAWWTF within the time frames specified in paragraph 1.3.1.1. Email address will be provided upon awardance of contract.
1.3. Statement of Work. Contractor shall provide the following services in support of 1 SOCES/CEOIB HFAWWTF:
1.3.1. Required Sample Analysis and Frequency
1.3.1.1. Daily Reclaimed Water. Samples shall be collected four times/week (ref 1.2.3.), between the hours 0800-1200, unless otherwise notified by a HFAWWTF representative. Testing and analysis shall be performed by contractor.
Sample results and completed COC shall be provided within 10 business days of sample pick up. Appendix A lists the “Daily Reclaimed Water Analysis” requirements.
1.3.1.2. Effluent and Influent Weekly Testing. Samples shall be picked up on
Tuesday, between the hours of 0800-1200. Testing and analysis shall be performed by contractor. Sample results and completed COC shall be provided within 14 business days of sample pick up. Appendix A lists the “Weekly
Influent/Effluent Analysis” requirements.
1.3.1.3. Quarterly Effluent Testing. Sample shall be picked up as part of the weekly Sampling 0800-1200. Testing and analysis will be performed in March, June, September and December by contractor. Sample results and completed
COC shall be provided with 10 business days of sample pick up. Appendix A lists the “Quarterly Effluent Sample Analysis” requirement.
1.3.1.4. Annual Groundwater Monitoring Well Sampling and Field Analysis.
Sampling and field analysis frequency shall be performed annually during the month of November. Sampling and field analysis shall be in accordance with
Florida Department of Environmental Protection (FDEP) Standard Operating
Procedures. Field Purge Log (DEP-SOP-001/01 Form FD 9000-24) for each groundwater monitoring well shall be provided with sample results. Sample results and completed COC shall be provided within 14 business days of sample pick up.
Appendix A lists the “Quarterly Groundwater Monitoring Well Analysis” requirements.
1.3.1.5. Annual Wetland Analysis. Samples shall be picked up annually in either
May or June. Testing and analysis shall be performed by contractor. Collection and preservation of samples will be conducted by HFAWWTF personnel. Sample pick up date will be coordinated between the contractor and a representative of
HFAWWTF. Sample results and completed COC shall be provided within 20 business days of sample pick up. Appendix A lists the “Annual Wetland Analysis” requirements.
1.3.1.6. Annual Digester Sludge Analysis. Sample shall be picked up in
January. Testing and analysis shall be performed by contractor. Sample pick up date will be coordinated between the contractor and representative of HFAWWTF.
Sample results and completed COC shall be provided within 20 business days of sample pick up. Appendix A lists the “Annual Digester Sludge Analysis” requirements.
1.3.1.7. Annual Residual (Sludge). Sampling shall consist of seven samples for analysis of Fecal Coliform Bacteria and commence in January. Sampling will be performed by HFAWWTF personnel. Testing and analysis shall be performed by contractor. Sample pick up date will be coordinated between the contractor and representative of HFAWWTF. Sample results and completed COC shall be provided with 7 business days of sample pick up. Appendix A lists the “Annual
Residual (Sludge) Analysis” requirements.
1.3.1.8. Additional Analysis. Additional testing and analysis shall be required under this contract and will be added as each testing and analysis is needed. Cost per test shall be the same as charged under regular testing and analysis.
Cryptosporidium/Giardia sampling conducted by HFAWWTP personnel shall only be utilized in the collecting the sample for delivery to lab. HFAWWTP personnel will not install, set-up, and/or maintain equipment for the collection of sampling of Crptosporidium/ Giardia, the lab may do so, with their contracted personnel and equipment, at no additional cost, than quoted in the schedule .
1.4. Performance Time. Sample results shall be completed in accordance with the requirements in paragraph 1.3 and shall be emailed or hand delivered to HFAWWTF. Email address will be provided upon awardance of contract.
1.5. Inspection. Personnel of HFAWWTF shall serve as the on-site representative for the Contracting Officer.
2.0 SERVICES SUMMARY
2.1. Service Summary (SS). The service requirements are summarized into performance outcomes that relate directly to mission essential items. The performance thresholds describe the minimum acceptable levels of service required for each requirement. The thresholds are critical to mission success and acceptable (satisfactory) performance.

3.1. Government-Furnished Services
3.1.1. Security Forces. The Government will provide general security service.
Security Forces phone extensions are 911 for emergencies, and 850.884.7114 for routine calls.
3.1.2. Emergency Medical Services. In the event of a severe emergency, 911 should be contacted. Ambulance transport responds from the Hurlburt Field Clinic Monday through Friday. On Weekends and Holidays, Okaloosa County ambulance transport will respond. One of the units will respond to the 911 call, and if necessary, transport a contractor employee to one of the local hospitals. The contractor shall reimburse the respective agency for these services.
4.0 GENERAL INFORMATION
4.1. Quality Control . The contractor shall provide a Quality Control Plan; in compliance with FAR 52.246-4 Inspection of Services - Fixed-Price.
4.1.1. Contents. The plan shall include a detailed description of the processes to be used during performance to ensure the services meet or exceed the requirements of the SS, the PWS and contract. The plan shall systematically provide for early identification of nonconforming services; develop metrics to track performance trends; detail corrective actions required to insure timely and acceptable performance in accordance with the PWS;
identify organizational placement of the inspectors; and describe the contractor’s partnering approach with the Government to ensure mission objectives are met. Additionally, plans that require employees to comply with the government control procedures for materials, equipment, facilities, and keys or lock combinations shall be included. 4.1.2. The contractor shall not employ persons for work on this contract if such employee is identified to the contractor by the Contracting Officer as a potential threat to the health, safety, security, general well being, or operational mission of the installation and its population. Where reading, understanding, and discussing safety, security, mission, and environmental issues are an integral part of a contract employee’s duties, that employee must be able to understand, read, write, and speak English.
4.1.3. The contractor shall not employ any person who is an employee of the US Government if employing that person would create a conflict of interest.
Additionally, the contractor shall not employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval according to Department of Defense (DoD) Regulation 5500.7-R, Joint Ethics Regulations (JER). The contractor shall not employ any person who is an employee of the Department of the Air Force if such employment would be contrary to the policies in AFI 64-106, Air Force Industrial Labor Relations Activities.
4.1.5. Dress/Appearance. Contractor personnel shall present a clean and neat appearance. Contractor shall not permit cut off shorts and clothing with tears, or revealing clothing or clothing with obscene or inflammatory designs, slogans, or remarks, and clothing representing military attire such as Battle Dress Uniforms or Desert Camouflage Uniforms. The contractor shall ensure personnel wear appropriate clothes suited for their job.
4.1.6. Smoking, Eating and Drinking. The HFAWWTF supervisor shall permit smoking, eating and drinking only in designated areas.
4.1.7. Driving License/On-base Requirements. The contractor personnel shall comply with base traffic regulations. The contractor shall ensure employees have a current and valid state/country driver’s license.
4.2. Place of Performance. The Contractor shall perform the required support at HFAWWTF located at 301 Downs Road, Hurlburt Field AFB, Fl, 32544.
Contractor personnel shall comply with all installation and facility safety and security regulations, as well as, the health, safety, and security provisions of the contract. Contractor personnel shall report security or safety problems to the Contracting Officer and/or Security Police as appropriate.
4.3. Safety. Contractors are solely responsible for compliance with Occupational Safety and Health Administration (OSHA) standards and the protection of their employees.
4.4. Conservation of Utilities. The contractor and their employees shall operate under conditions that prevent the waste of utilities.
5.0 CONTRACTOR PERSONNEL
5.1. CONTRACTOR/EMPLOYEE BASE PASS AND IDENTIFICATION,
SPECIAL CLEARANCES, AND VEHICLE PASSES. The contractor shall comply with all requirements and procedures IAW AFFARS 5352.242-9000, Contractor Access to Air Force Installations and other requirements stated herein. The contractor shall obtain base identification and vehicle passes, if required, for all Contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor employees are required to wear or prominently display installation identification badges or contractor-furnished contractor identification badges while visiting or performing work on the installation.
5.1.1. When reporting to the registration office, the authorized contractor individual(s) shall provide a valid driver’s license, current vehicle registration, and valid vehicle insurance certificate to obtain a vehicle pass. Hurlburt Field adheres to the guidelines of the REAL ID Act outlined in Air Force Guidance Memorandum to AFMAN31-113.
5.2 THE CONTRACTOR EMPLOYEE LIST. This form shall list every employee requiring base access. Every time employees change, it shall be updated to reflect additions and/or deletions. Identify those individuals you wish to have escort authority by including the word “escort” or “E” next to their names. Limit the number of individuals designated as escorts to a reasonable number (i.e. one or two). For work locations use building numbers or specify as base wide if applicable. Once completed, email the list to the Contracting Office representative in EXCEL format. The Contracting Officer will then prepare, attach a cover letter, and forward the Employee List to the Visitor Control Center.
The contractor shall submit the employee listing the Contracting Officer no less than three days prior to the performance start date and at least one day prior as changes occur.
5.2.1. Upon notification of the employee listing being forwarded to the Visitor Control Center (VCC), all contractor employees shall report to the VCC to complete the required documentation for base access. Contractor employees shall be required to provide two (2) forms of identification IAW paragraph 5.1.1.
If an employee will be driving on Hurlburt Field, they are required to provide a valid driver’s license, vehicle registration, and proof of insurance. NOTE: The section of Commercial gate Control Center administrating this function is closed after 1800 (6:00 p.m.), therefore, employees must arrive prior to this time in order for their documentation to be processed.
5.2.1.2. During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site.
5.2.1.3. Upon completion or termination of the contract or expiration of the identification passes, the contractor shall ensure that all base identification passes issued to employees are returned to the issuing office.
