AIRFIELD & HEAVY DUTY

STATEMENT OF WORK

FY25 AIRFIELD PAVEMENT PREVENTATIVE MAINTENANCE

PART I – PROJECT SPECIFIC GUIDANCE

1. SCOPE

1.1 OBJECTIVE

The project consists of providing all labor, materials and equipment necessary to perform spall repair, crack sealing & repair in several airfield locations (see location below).

1.2 PERIOD OF PERFORMANCE:

Total period of performance (POP) for this project is 365 days from the date of the Notice to Proceed (NTP).

1.3 LIMITATIONS:

The Contractor must work with Security Forces, Civil Engineering Program

Management and Airfield Operations to coordinate work at this site. The contractor should assume the airfield would remain active during the repair process. M u l t i p l e m o b i l i z a t i o ns m a y b e r e q u i r e d t o a c c o m p l i s h a l l w o r k t a sk s .

Contact with Ground Control will be necessary for most of the work.

1.5 DESCRIPTION OF WORK:

A. Crack seal 30,000 LF of asphalt pavement

B. Provide and install 4146 LF preformed elastomeric joint seal

C. Repair defective manhole structures/covers

D. Excavate, as necessary, to support other repair tasks.

E. Repair 1000 SF-IN of concrete spalls

F. Pave with 2 inches of FAA Mix using up to 5 Tons FAA Mix (PG 76-22 binder).

G. Reseed areas disturbed by earthwork excavation.

1.6 POINTS OF CONTACT (POC):

1.7 GOVERNMENT-FURNISHED INFORMATION, EQUIPMENT, MATERIAL, AND

PROPERTY

A. INFORMATION – The Government may provide information in the form of available plans, reports, maps, and drawings as necessary to facilitate accomplishment of the work required in this Statement of Work. The Government may furnish electronic copies of available As-Built drawings that are pertinent to the buildings involved. However, these drawings are not warranted to show conditions as they exist. The designer shall field verify the information shown on the As-

Built drawings as necessary to produce a quality-designed project.

B. MATERIAL – None provided.

2. DESIGN REQUIREMENTS (N/A)

3. CONSTRUCTION REQUIREMENTS

A. In accordance with (IAW) United Facilities Guide Specifications (UFGS) 01 33 00

Submittal Procedures shall govern construction submittals.

B. PERMITS – The Contractor is responsible for all applicable permits, licenses, and certificates required to perform and execute this contract.

C. CONSTRUCTION PROGRESS MEETINGS -Not Required

4. SECURITY

4.1 SECURITY INSTRUCTIONS

a. All personnel employed by the contractor in the performance of this contract, or any representative of the contractor entering the Installation shall abide by all security

b. At the close of each work period, government facilities, equipment and materials shall be secured, lights, heat and water turned off and all doors and window secured.

c. The contractor and, as applicable, subcontractor shall not employ persons for work on this contract if such employee is identified as a potential threat to the health, safety, security, general wellbeing or operational mission of the Installation and its population, nor shall the contractor or subcontractor employ persons under this contract who have an outstanding criminal warrant as identified during the Criminal Background

Check (CBI).

d. Contractors shall ensure their employees and those of their subcontracts have the proper credentials allowing them to work in the United States. Employees and subcontractors later found to be undocumented workers will be remanded to the proper authorities.

e. All contractors and subcontractors when working in a Controlled, Restricted, or other sensitive area must be escorted at all times. The military agency or unit responsible for the project is responsible for providing the escorts. The Contractor shall follow existing procedures and instructions for obtaining entrance to restricted or controlled areas.

4.2 CONTRACTOR BADGE POLICIES:

All requests for Contractor badges will be submitted by a “Sponsor.” Typically, this will be the Base Contracting Squadron (436 CONS), a Contracting Officer Representative (COR) or the Requirement Owner (RO). The “Sponsor” will complete an Operational Risk

Management Assessment Form (ORM) and submit to the 436 SFS, Pass and Registration office for approval IAW OPlan 31-101. As a minimum the ORM will be submitted 10-days prior to the requested date of employment.

a. All Contract employees granted unescorted access, to include Access Control Lists

(ACLs), are required to consent to a Criminal Background Investigation (CBI) prior to entry to the Installation. Continued employment is contingent upon successful completion and favorable reporting of the CBI. The Contractor shall provide the following information prior to entry for sponsorship:

• Requests for Access Control Lists (ACL) must be submitted/approved NO LATER

THAN SEVEN (7) FULL BUSINESS DAYS before access is required. This allows for adequate time to properly screen personnel and obtain follow-up information if needed.

All access control lists requests must include the sponsor’s information. The sponsor must provide the following information on all guests: Last Name, First, MI, (no nicknames) in alphabetical order, Date of Birth and Driver’s License Number with State of Issue. All

ACLs should be submitted via email to 436 SFS/EAL (In the Global Address Book)

(436sfs.eal@us.af.mil).

• ACLs will be for visits under 60 days. The sponsor must be specific on the following.

Reason for Requested Access, Event Location: Provide the building number or the individual’s residence address. All Access Control Lists must be in a Word Document that is attached to the email. Requests submitted in a PDF format or in an Excel document will be returned for correct formatting. Do not paste the Access Control List in the body of the email.

• When requesting access for sub-contractors the following information is needed: full name, Social Security Number, driver’s license number and state of issue, vehicle information (make/model/color/year), license tag number and state, name of company, date and time required for access.

b. Contractor badges will be issued for a maximum period of one year. Prior to re-issuing a badge, all old badges must be returned to the 436 SFS, Pass and Registration office for destruction. Prior to re-issuing a badge, the “Sponsor” must submit a new ORM to the 436 SFS, Pass and Registration office for approval. In the event a badge is lost or stolen, the badge holder must immediately contact the 436 SFS, Pass and Registration office. The badge holder must complete the Loss/Theft of Identification Worksheet and provide to the Contractor’s Site Supervisor. The Contractor’s Site Supervisor will investigate the loss and report (in writing) the circumstances in which the badge was lost. This information, along with a new ORM, will be submitted by the “Sponsor” to the 436 SFS, Pass and Registration office for re-issuing a badge.

c. When a Contract employee is terminated, the Contractor’s Site Supervisor, COR or RO will immediately notify 436 CONS, who will ensure notification is made to the 436

SFS, Pass and Registration office. The Contractor’s Site Supervisor, COR, or RO are responsible for returning the contractor’s badge to the 436 SFS, Pass and Registration office. If a terminated employee refuses to surrender the access badge, COR or RO will immediately notify, in the order of precedence: (1) 436 SFS, Base Defense Operation

Controller; (2) the 436 SFS, Pass and Registration office; and (3) 436 CONS of the situation. If a base access badge is denied the 436 SFS, Pass and Registration office will notify the “Sponsor” of the denial. The individual in question may contact the 436 SFS, Pass and Registration office for information on how to obtain the reason(s) for denial.

4.3 INSTALLATION ACCESS: The contractor shall obtain a personal contractor identification badge for all employees and vehicle passes for all contractor and personal vehicles requiring entry onto Dover AFB from 436 SFS, Pass and

Registration, for the duration of the contract. Employees are only permitted to enter the Installation during the date and time periods indicated on their contractor badge.

a. Vehicle registration, proof of insurance, and a valid driver’s license must be presented for all vehicles while operating on the Installation. All vehicles entering the DAFB

Main Base, Military Family Housing or sensitive areas are subject to search. Any refusal or non-consent by an employee will result in termination of their base access and immediate confiscation of this access badge.

b. During Force Protection Condition (FPCON) Normal and Alpha, personnel without base issued badges must be sponsored onto the installation. During FPCON Bravo, Charlie, and Delta personnel without base issued identification shall be physically escorted onto the installation.

4.4 ANTITERORISM:

Defense Federal Acquisition Regulation Supplement (DFARS) (Reference (r)) reflects current DoD AT security requirements for defense contractors.

Joint Publication 3-07.2 Antiterrorism

Terrorism is defined as: The calculated use of unlawful violence or threat of unlawful violence to inculcate fear; intended to coerce or to intimidate governments or societies in the pursuit of goals that are generally political, religious, or ideological.

Antiterrorism: As a prelude and during performance of any contract, it is critical that the

Contractor be supportive of all Department of Defense (DoD) requirements to protect personnel and their families, installations, facilities, information, and other resources from terrorist acts.

Antiterrorism Awareness Guides: All personnel employed by the contractor, to include subcontractors, shall review the attached DAFB AT Awareness Guide for Contractors (Apr 24) and DAFB AT Awareness for Contractors Trifold Apr 24.

Contractors or subcontractors possessing a Common Access Card (CAC) will complete AT

Awareness Level I training IAW AFI 31-145, Antiterrorism, Standard 25, by completing the―Force Protection computer based training (CBT) course through the MyLearning site.

This training site is available through the AF Portal at https://lms-jets.cce.af.mil/moodle/. This site automatically records members completion of training and enables the user to print a certificate if needed by the Unit or Base Training Manager.

Contractors or subcontractors without a CAC or who do not have access to a government Non-classified Internet Protocol Router Network (NIPRNET) computer performing construction/services on the installation will complete AT Awareness Level I training IAW AFI

31-245, Antiterrorism, Standard 25, by completing the― Force Protection computer based training (CBT) course through the Joint Knowledge Online (JKO) Learning Management

System (LMS) at http://jko.jten.mil/courses/atl1/launch.html

4.5 FORCE PROTECTION CONNDITION (FPCON) IMPACT ON WORK LEVELS

a. If the DAFB FPCON is elevated to Charlie, the installation shall evaluate the need for contract services to be discontinued. At FPCON Delta, services are discontinued.

Services shall resume when the FPCON level has been lowered.

b. Administrative Note: In the event of an emergency, dial 911 from any land line on the installation. The call will be routed to the ECC (302-677-6666). If a cell phone is used to dial 911, it will be routed to the county 911 dispatch. Inform the dispatcher the emergency is on DAFB, and they will route the call to the DAFB ECC.

A. FLIGHT LINE DRIVING:

IMPORTANT:

1. Vehicle Operations: Individuals that will operate a vehicle on the airfield shall receive driver's training from Airfield Management prior to starting work. Careful attention and strict adherence to the training will prevent accidental damage to aircraft and possible injury to both flight and ground personnel. The training should be completed ASAP but NLT 24 hours prior to the start of the project. Contact 302-677-4183/4185 to schedule a training time.

2. Vehicle operations within the controlled movement area (CMA) are under the control of tower personnel. Control tower radio instructions shall be observed and obeyed by vehicle operators at all times while performing work on the airfield. The Contractor shall assign the

Government furnished radio to one of his personnel on the airfield whose responsibility shall be to maintain contact with control tower personnel and coordinate all Contractor movement within the CMA. The contractor shall check in with Airfield Management each morning, to state the location and type of work to being performed on the airfield and at the end of the day to advise Airfield Management work is completed for the day. Airfield

Management will need to check the construction site for compliance with UFC, ETL and

AFI for construction site marking, lighting, obstruction clearance and Foreign Object

Damage (FOD) potential.

3. Airfield Barricades: Barricades shall comply with Unified Facility Criteria (UFC) and

Air Force Instructions (AFI). Barricades shall consist of reflective orange and white low-profile barriers listed in Unified Facility Criteria Engineering Technical Letters (UFCETLs) or AFIs. Maximum spacing of barricades is 50 feet, with two amber-yellow lights on each barricade. Spacing requirements are found in the UFCs. All barricades shall be either anchored or of sufficient mass to retain an established position where they are placed, in all types of weather conditions. Flashing lights shall be at least 5 candelas effective intensity and flash at a rate of 55 to 75 flashes per minute. Continuous burning lights must have an effective intensity of 10 candelas.

4. Contractor will be provided a copy of UFC 3-260-01 attachment 14 for Construction

Phasing plan and operational safety on airfields during construction.

5. During construction there will be times, due to mission requirements, the contractor will not be authorized to work on site. The times could verify from 24 to 48 hours for work stoppage.

5. CONTRACT ADMINISTRATION REQUIREMENTS

A. PROGRESS SCHEDULE, AF FORM 3064 – The Contractor shall prepare and submit, for approval, by the Contracting Officer (CO), a work breakdown structure. The work breakdown structure shall be used to report the cost and schedule status for this project. All tasks required under this type of contract shall be included in the work breakdown structure.

B. PROGRESS REPORTS, AF FORM 3065 – The Contractor shall complete biweekly Contract Progress Reports throughout the contract period. The work elements and percentages of the total job identified on the AF Form 3065 shall be identical to the work elements and percentages indicated on the AF Form 3064.

Each invoice should be based on the most recent approved AF Form 3065 and will be entered through Wide Area Workflow (WAWF). The AF Form 3064, AF

Form 3065, and invoice shall all cover the same period of time.

C. NOTIFICATION REQUIREMENT – The Contractor is required to notify the

Contracting Officer and the Project Manager of critical issues that may affect life, health, safety, contract performance, or the environment. Verbal notification should be made immediately followed by written notification as soon as practical.

D. TRANSMITTAL FORM – The Contractor is required to use AF Form 3000

Material Approval Submittal as the transmittal form for both design and construction submittals.

E. SUBMITTAL FORMAT – Provide submittals in electronic format, with the exception of material samples required for Submittal Description (SD)-04

Samples items. In addition to the electronic submittal, provide one hard copy of design submittals. Compile the submittal file as a single, complete document, to include the Transmittal Form described within. Name the electronic submittal file specifically according to its contents, coordinate the file naming convention with the Contracting Officer. Electronic files must be of sufficient quality that all information is legible. Use Portable Document Format (PDF) as the electronic format, unless otherwise specified or directed by the Contracting Officer.

Generate PDF files from original documents with bookmarks so that the text included in the PDF file is both searchable and can be copied. If documents are scanned, Optical Character Resolution (OCR) routines are required. Index and bookmark files exceeding 30 pages to allow efficient navigation of the file. When required, the electronic file must include a valid electronic signature, or scan of a signature. Email electronic submittal documents fewer than 10MB to an email address as directed by the Contracting Officer. Provide electronic documents over 10MB on an optical disc, or through an electronic file sharing system such as the AMRDEC SAFE Web Application located at the following website:

https://safe.amrdec.army.mil/safe/. Provide hard copies of submittals when requested by the Contracting Officer. Up to three additional hard copies
