About this file
This document is a Contract Discrepancy Report (CDR) template used by the Contracting Officer's Representative (COR) to formally document and address contractor performance issues.
The CDR template establishes a standardized process for identifying and resolving contract performance deficiencies. It requires completion of the following sections: contract number, contractor task manager or on-site representative name, COR name, date and time the discrepancy was observed, and a detailed description of the discrepancy or problem with any supporting attachments. The template includes a determination of whether a written corrective action plan is required and, if so, specifies the number of days (to be inserted) within which the contractor must submit the corrective action plan to the COR. The document must be signed by both the COR who prepared it and the contractor's task manager or on-site representative upon receipt. The COR may initiate a CDR whenever monthly recorded defects for a performance standard exceed allowable limits, when unacceptable performance is determined to be critical in nature requiring formal corrective action, or whenever an unfavorable trend in contractor performance is detected.
