About this file

This is an Insurance Requirements Checklist for the Solid Waste Authority of Palm Beach County in Florida related to a term contract (Agreement No. 27-01/AL) for the disposal and recovery of waste oil, oily water, antifreeze, petroleum contact water, and used filters from seven hazardous waste facilities. The contract operates on an as-needed basis to support Authority operations. The document, prepared on October 26, 2026, and revised on September 21, 2026, specifies comprehensive insurance requirements that vendors must maintain before accessing SWA property or commencing contract activities. Key insurance coverages required include Commercial General Liability with minimum limits of $1,000,000 per occurrence and $2,000,000 general aggregate, Automobile Liability at $1,000,000 combined single limit per accident, Workers' Compensation at statutory limits, Pollution Liability/Environmental Impairment Liability or Contractor Pollution Liability at $1,000,000 aggregate, and various additional coverages such as Employer's Liability and Fire Legal Liability. All certificates of insurance must include a Waiver of Subrogation endorsement and name SWA as the Certificate Holder with current dating.

All insurance policies must use current ISO forms or SWA Risk Management-approved alternatives that are no more restrictive than the most recent ISO forms. The Solid Waste Authority of Palm Beach County, its Officers, Employees, and Agents must be named as Additional Insureds where permitted by law using endorsement forms CG2010 and CG2026, except for Workers' Compensation, Employer's Liability, Professional Liability, and Inland Marine coverage. The prime firm bears full responsibility for ensuring compliance with these insurance requirements, which extend to all employees and subcontractors. Vendors must provide complete policy documentation upon request, with all endorsements listed by form name and number. Self-insured programs or any coverage exceptions require prior approval from SWA Risk Management. The document does not indicate any small business set-asides, pricing terms, or incumbent contractor information.
