About this file
This is a reference questionnaire issued by the Nevada State Purchasing Division, Department of Administration, for solicitation number 08DOA-S4023 related to janitorial services for various agencies. The reference questionnaire is designed to gather feedback from current and former clients regarding vendor performance on projects of similar size and scope. The document is directed to clients providing references for proposing vendors or subcontractors. The completed questionnaire must be submitted to Annette Morfin, Purchasing Officer II, at amorfin@admin.nv.gov no later than November 9, 2026, with the solicitation number included in the subject line. The underlying contract opportunity involves janitorial services for the Nevada Department of Motor Vehicles at seven locations in Las Vegas: Decatur, Flamingo, Sahara, Silverado, Silverado CDL, Campos, and Fire Marshal buildings.
The reference questionnaire requires clients to evaluate vendor performance across thirteen rated criteria using a scale of 0-10, where 0 indicates poor or inadequate performance, 1-3 represents below average performance, 4-6 indicates average performance, 7-9 represents above average performance, and 10 indicates exceptional performance. Evaluation categories include vendor knowledge and expertise, flexibility regarding scope and timeline changes, satisfaction with delivered materials and products, staff interaction dynamics, schedule adherence and timeliness of deliverables, customer service responsiveness, billing accuracy and timeliness, problem resolution capability, meeting business requirements, local office response time, and likelihood of future recommendations. Responses designated as confidential business information are protected from public disclosure pursuant to Nevada Revised Statutes. All questions must be answered, with "U/K" used if information is unknown and "N/A" if not applicable; additional pages may be attached as needed.
