About this file

This is a Bidder Declaration form issued by the California Department of General Services, Procurement Division, for use in response to IFB 26-3033 from the California Department of Social Services. The form requires all prime bidders to declare their California state certifications, including Microbusiness (MB), Small Business (SB), Nonprofit Veteran Service Agency (NVSA), and Disabled Veteran Business Enterprise (DVBE) status. Prime bidders must indicate whether subcontractors will be used and identify the distinct elements of work to be performed by the prime bidder versus subcontractors. The declaration specifically addresses whether prime bidders with DVBE certification act as brokers or agents and whether they own at least 51 percent of rental equipment provided under the contract.

If subcontractors are proposed, the form requires completion of a detailed table listing each subcontractor's name, contact information, address, email, California certification status, work to be performed, percentage of bid price, good standing status, and rental equipment ownership percentage where applicable. All certified subcontractors must provide a commercially useful function as defined under Military and Veterans Code Section 999 for DVBEs and Government Code Section 14837(d)(4)(A) for small and microbusinesses. Prime bidders must verify that subcontractors possess valid licenses, are qualified to do business in California, and maintain good standing with the State of California Secretary of State. Failure to properly complete this declaration or to ensure that certified bidders provide commercially useful functions may result in rejection of the bid as non-responsive.
